Family and Children's Services has a long legacy of serving Union County and the surrounding areas. From providing trauma-informed mental health therapy, to hosting community workshops on trauma-related topics, to offering in-home life skills guidance, Family and Children's Services has been coaching, supporting and encouraging vulnerable individuals and families since 1893.


JOB FUNCTION:

Provide culturally competent, strength-based non-therapeutic family case management services to a variety of Agency clients in order to strengthen and preserve families by providing, family-centered support and case management services.


JOB EXPECTATIONS:

Supervision Requirements:

  • Meets with supervisor in accordance with program requirements.
  • Utilizes supervision effectively to address client-related issues.
  • Utilizes supervision effectively to address performance issues and concerns.
  • Utilizes supervision effectively to explore professional development and training needs.

Program Quality Requirements:

  • Complete client/family-centered assessments, service plans, progress notes, treatment plans, progress updates, and other required reports
  • Drive clients to and from appointments when required
  • Maintain electronic case record using the MyEvolv system
  • Ensure quality and accuracy of written and verbal communications
  • Promote the safety and well-being of children, family members, and community
  • Maintain and build upon primary family connections
  • Help parents improve their parenting abilities, identify parental strengths, and support their efforts to care for their children
  • Prevent, reduce, or eliminate barriers, institutional practices, and community conditions that may place a child, family, or community at risk
  • Further the creation of a stable and nurturing family environment in which children can grow and develop
  • Improve individual and family conditions within the context of their culture and community
  • Identify and connect with community resources needed to improve family conditions
  • Attend Case Conferences with DCPP as necessary
  • Educate self on issues impacting the program population
  • Follows program and agency policies & procedures
  • Complete weekly schedules and time sheets (via People Strategy) per established guidelines
  • Submit Level of Service (LOS) within program requirements
  • Comply with the ethical standards and license requirements of respective professions
  • Comply with state confidentiality laws and child abuse reporting laws
  • Provide Treatment Summaries to referring agencies in a timely manner
  • Comply with Utilization Review Criteria for Admission, Continuation, and Completion of Services
  • Conduct self in a professional manner that positively reflects the agency culture
  • Work collaboratively with coworkers and agency staff to effectively communicate and problem-solve
  • Attend agency and program meetings and trainings
  • Play an active role in meetings and trainings designed to enhance agency and program operations
  • Must be able to analyze work, set goals, develop plans, and utilize time effectively and efficiently
  • Reasonable accommodations may be made if necessary to enable individuals with disabilities
  • Other duties as assigned to perform the essential functions

QUALIFICATIONS & REQUIREMENTS:

  • Bachelor's degree in social services or related field
  • A minimum of one year of experience providing case management services to children/adolescents/families, exclusive of internships
  • Some experience facilitating/co-facilitating groups or comfort with public speaking
  • Good communication skills, both verbal and written
  • Working knowledge of electronic health record system
  • Bilingual/Spanish-English required
  • Some evening and Saturday hours required
  • Must have valid driver's license with good driving record
