Inpatient Case Manager RN

Sentara Healthcare is seeking an experienced Registered Nurse to join our Inpatient Case Management team as a Resource Pool RN. This role is responsible for providing comprehensive care management services to patients across the healthcare continuum, with a focus on patient transition to the appropriate level of care.
Responsibilities- Assess patients' social, psychological, cultural, environmental, and financial situations to identify needs and resources.
- Develop and evaluate effective transition plans, taking into consideration patients' diagnoses, prognoses, treatment plans, and available healthcare benefits.
- Collaborate with patients, families, healthcare teams, and community organizations to ensure seamless care transitions.
- Provide education and support to patients and families to promote self-management and independence.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.
- 18 months of recent inpatient RN Case Management experience.
- Current RN licensure in North Carolina or Virginia.
- Basic Life Support (BLS) certification within 90 days of hire.
- BSN preferred; all RNs without a BSN will be required to sign a BSN agreement committing to successfully obtain their BSN within 5 years of hire.
- Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification required within one year of eligibility.
