The Care Coordinator–Substance Use Disorders provides outreach and service coordination for referred patients diagnosed with substance use disorders. As part of a recovery oriented system of care, the Care Coordinator–Substance Use Disorders works in collaboration and partnership with identified patients, family/caregiver(s), providers and community resources in a team approach to promote recovery of the patient.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: High school diploma or equivalent.
Preferred/Optional: Bachelor’s degree in a human services or social services area.
EXPERIENCE
Minimum Required: Personal and/or professional knowledge of substance abuse and integrated service delivery systems. Experience with communication, facilitation, organizational skills and the ability to multi-task. Must be PC literate, with a working knowledge of word processing.

CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: State of Wisconsin driver’s license with an acceptable driving record. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
