Job Summary

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at DaVita IKC. As a key member of our care team, you will play a vital role in facilitating the planning and organization of critical activities for our patients. With a focus on delivering high-quality care, you will work closely with our Nurse Practitioners, RN Case Managers, and other healthcare professionals to ensure our patients receive the best possible care.

Responsibilities
  • Complete non-clinical screenings and escalate as needed.
  • Help identify eligible resources for patients and navigate resources to support their care.
  • Assist with the referral process and coordinate provider appointments.
  • Support the clinical team in moving patients through the continuum of care by communicating with and completing tasks as directed.
  • Assist the care management staff in implementing and documenting discharge care plans, including initiating referral forms, gathering clinical information, and scheduling appointments.
  • Communicate directly with patients on a timely basis for all scheduling requirements.
  • Maintain accurate and complete documentation of records, ensuring all forms are fully completed, legible, and have the proper signatures.
  • Ensure that documents are distributed to the appropriate parties promptly, without compromising confidentiality or patient privacy procedures.
  • Demonstrate exceptional two-way communication skills using clear and well-organized oral and written techniques.
  • Act in accordance with DaVita's mission and values to deliver high-quality care to our patients.
Requirements
  • High school diploma or GED required.
  • At least two years of experience in a professional office environment.
  • Ability to work with and resolve confidential information and/or issues using discretion and judgment.
  • Excellent verbal communication skills over the phone.
  • Proficiency with Microsoft Office/Excel/Outlook.
  • Commitment to service excellence and patient satisfaction.
  • Excellent verbal and written communication skills.
  • Ability to work autonomously.
Preferred Qualifications
  • 1-3 years of clinical medical office experience.
  • CNA, MA experience is a plus.
  • Experience coordinating care for patients, including referrals, ACO care management, and navigating insurance payors.
  • Experience working in direct patient care/support.
  • Experience working in a medical office environment.
