Korzen Health is working with the partnership of Healogics to find a Post Acute Nurse Practitioner.

We are looking to add an experienced NP to cover local SNFs 5 days per week. Procedure experience within the last 2 years IS required. For example: sutures, central lines, intubations, and anything that would be very specialized.

This is a full time, 5 days/week position - flexible hours. Post-Acute Care wound care program located in Lafayette/Kokomo/Logansport, IN market. This is 100% wound care position. 52 hours of paid training included.

This is a mobile position. We have 5 days of work in Lafayette/Kokomo/Logansport, IN market area. You will be traveling to skilled nursing facilities (SNFs) to provide wound care to patients.

Scope of Practice Includes:

  • You will average around 18-20 patients a day.
  • Performing basic wound care (dressing changes, chemical debridement) with some minor procedures.
  • This will be a full-time 5 days a week job. No Nights, No Call, No weekends. Flexible.
  • Compensation Model is salary with RVU after 1 year.
  • You will work with Adult and Geriatric patients.
  • Opportunity for growth within leadership.

Wound Care Nurse Practitioner and Experience Requirements

  • State License
  • DEA License
  • SNF or Wound Care Exp a Plus
  • ED or Urgent Care experience is a plus
  • 1 year of NP experience is a must

Benefits Include

  • Compensation is a mixture of Solid Base Salary & Quarterly Bonus. Base+Bonus=Range of $110K-$155K/year
