ABOUT THE ROLE
*Schedule: Monday-Friday 8:00am-5:00pm
*Territories: Salt Lake City Area, Murray, Payson
- The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Sage Hospice and Palliative Care either directly or through coordination of care with other counselors.
HOSPICE CHAPLAIN ROLE REQUIREMENTS
- Associates degree in divinity, theology, or biblical studies or certification as an ordained minister.
- One (1) year of experience in hospice or healthcare.
- In Georgia only: documented completion of Clinical Pastoral Education (CPE), per state requirements.
- Experience with patients, families/caregivers through death or dying.
- Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.
- As a Bereavement Coordinator, you're responsible for planning, implementing and maintaining a bereavement program to meet the needs of family members and/or caregivers for up to one (1) year following the transition of a hospice patient.
BEREAVEMENT COORDINATOR REQUIRED EDUCATION/EXPERIENCE:
- Bachelor’s degree in social work or theology or related experience.
- One (1) year experience in clinical grief counseling and response with patients, families, or caregivers through death/dying issues.
- Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.
PREFERRED
- 1-2 years of experience as a Chaplain
