Reports To: Palliative Care Practice Manager

Status: Non-exempt (Hourly)

FT, PT, PRN: Full-time

Shift: 40 hours; Monday - Friday; Varied Day time hours per patient needs

Location: Buncombe County, NC

Pay Range: $15.50 - $18.50 per hour

Position Summary:

Provides personal care and/or home management tasks to Palliative Care patients diagnosed with dementia/memory care, under the direction of a Memory Care RN.

Essential Duties & Responsibilities:
  • Practice within the scope of training, license, certification or registration, where applicable, and as required by law and regulation
  • Performs tasks as delegated to and directed by a registered nurse:
  • Practice safety and infection control methods
  • Provision of assistance with Activities of Daily Living &/or Instrumental Activities of Daily Living
  • Report to work punctually and appropriately dressed for role
  • Complete required orientation and on-going training to include modules assigned via Memory Care Caregiver College to meet the training requirements under the Guide Model

Knowledge, Skills & Abilities:
  • Knowledge and application of proper body mechanics
  • Knowledge and application of Infection Control Principles and environmental safety

Minimum Qualifications:

Education:

  • High School Diploma/GED

Experience:

  • One (1) year caregiving experience preferred

Licensures & Certifications: (if applicable)

  • Valid driver’s license and proof of automobile insurance
  • Maintain valid, good-standing listing on NCNAR if working as CNA

Summary of Physical Requirements:
  • Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
  • Flexion/Extension of arms/wrist
  • Hand dexterity to include gripping, pushing/pulling and writing
  • Ability to assist transfer/repositioning if assigned to hands-on clients

Other Duties:
  • Work to achieve applicable performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
  • Demonstrate continued competency by participating in ongoing education and training offered by the agency and/or outside resources as required for role.
  • Exhibit competency in the observance of Universal Precautions by applying principles of infection control by:
    • Knowing standard precautions
    • Washing hands before and after patient care
    • Wearing gloves and other PPE as necessary
  • Communicate any changes in Client condition or occurrences to Palliative Care RN
  • Maintain chart documentation of care provided in accordance with the written plan of care.
  • Maintain confidentiality and privacy of any PHI per HIPAA requirements
  • Maintain general order, safety and cleanliness of the Client area
  • Practice personal and environmental safety
  • Demonstrate proper transfer and lift techniques using good body mechanic techniques
  • Exhibit competency in Activities of Daily Living such as:
    • Assist in ambulation, transfers or exercises
    • Assist with bathing, dressing and grooming
    • Assist with feeding
    • Assist with toileting and incontinence care
  • Exhibit competency in Instrumental Activities of Daily Living such as:
    • Housekeeping
    • Meal preparation
    • Safe transportation
    • Medication reminders
    • Pet care
  • Practice professional boundaries and behaviors and communication
  • Other duties as assigned by registered nurse, clinical team leader, or Director/Manager
