About the Role

CarePartners is seeking a highly skilled Registered Nurse Home Care PACE to join our team of dedicated healthcare professionals. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Assess patients' physical, psychosocial, and environmental needs, and develop individualized care plans to address these needs.
  • Provide skilled nursing care to patients, including wound care, medication management, and education on self-care techniques.
  • Collaborate with the interdisciplinary team to establish and achieve patient goals, and maintain high-quality patient care.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided.
  • Participate in on-call rotation and emergency call according to agency policy.
Requirements
  • Current RN licensure in the state of North Carolina.
  • Minimum of 1 year of experience in home health care or a related field.
  • Strong assessment and communication skills.
  • Ability to work independently and as part of a team.
