Summary: The Community Services Case Manager will provide support and assistance to families of children and adults who are eligible for DDS services, including those on the autism spectrum. Services and supports include information and referral, service navigation, support groups, family training, parent networking, mentoring, facilitating social/recreational events, and transportation based on the priority needs of families. The Community Services Case Manager will assist in obtaining public benefits, entitlements, and generic resources available in the local geographic area. The Case Manager will be responsive to the cultural and linguistic needs of the identified families.

Schedule: 20 hours, flexible

Essential Duties and Responsibilities:

  1. Provide support and assist families with obtaining services to meet their priority needs.
  2. Create opportunities and provide support to individuals in making decisions to develop their skills, enjoy creative social experiences, and foster independence.
  3. Provide services to families and individuals according to DDS, CARF, all applicable regulatory standards, and Bridgewell policies and procedures.
  4. Provide support to families with medical, clinical, and financial needs.
  5. Transport individuals as needed to and from medical, recreational, and other community activities.
  6. Maintain required documentation according to DDS and all applicable regulatory standards.
  7. Assist families with accessing resources and recreational opportunities in the community.
  8. Facilitate management of flexible spending funds as needed.
  9. Data collection and reporting according to DDS and/or Bridgewell requirements.
  10. Maintain core training certifications as mandated by DDS and Bridgewell.
  11. Other duties as assigned.

Required Education/Experience:

  1. Must have a Bachelor's Degree in a related field or, in the alternative, three years' experience working in the Human Services field.
  2. A minimum of 1 year of experience working in the human services field and/or experience working with families of children or adults with intellectual and developmental disabilities including families of adults on the autism spectrum.

Required Skills/Knowledge:

  1. Excellent written and oral communication skills.
  2. Excellent organizational and problem solving skills.
  3. Valid driver's license and one year of driving experience.
  4. Must have a reliable vehicle that has the required insurance coverage.
  5. Basic computer skills.
  6. Bilingual/bicultural preferred especially in Spanish or Khmer.

Physical Demands:

  1. While performing the duties of this job, the employee is frequently required to climb stairs.
  2. Must be able to lift up to 20 pounds.
  3. The employee must be able to drive a vehicle.
  4. Must have a normal range of eyesight with or without corrective lenses.
  5. Must have a normal range of hearing with or without corrective equipment.
