As a Community Health Worker you will work collaboratively and under the direction of the Clinic Care Coordinator and Clinic Nursing Manager. In collaboration with the Clinic Care Coordinator and Clinic Nursing Manager, the Community Health Worker is responsible for healthy lifestyle coaching, education, training, and motivational support to patients, and to provide community resource management to encourage quality outcomes, smooth transitions, and coordination of care across the health continuum. The Community Health Worker encourages healthy lifestyle choices to prevent and reduce long term effects of chronic illness. This position is accountable to the Clinic Care Coordinator for working with and representing the primary care clinic across multiple constituents. Spanish speaking is preferred but not required.

You may qualify for this position with the following credentialing and experience:

  • High School Diploma or equivalent.
  • Active license in the State of Washington as a Medical Assistant-Certified, preferred. CNA or EMT certification considered.
  • Valid Washington State Driver’s License and reliable transportation.
  • Current (Healthcare Provider [HCP] level) CPR certification.
  • Experience or education with patient coaching, education, and training, preferred.
  • Completion of education program specific to the 7 domains of care coordinator in primary care or the ability to obtain within six months of hire.

Competencies include, but are not limited to:

  • Critical thinking skills: Seeks resources for direction, when necessary. Performs independent problem-solving.
  • Performs actions that demonstrate accountability. Exercises safe judgment in decision-making. Practices within legal and ethical guidelines.
  • Demonstrates competency to care for customers/patients across the age continuum.
  • Demonstrates a basic understanding of the essential function representative of Seven Domains of Care Coordination in the primary care setting: Population Health Management, Comprehensive Assessment, and Care Planning, Interpersonal Communication, Education/Coaching, Health Insurance and Benefit knowledge, Community Resource knowledge and Research and Evaluation skills.
  • Demonstrates competency on equipment listed

This is a part-time 32 hour per week position with availability Monday through Friday. 8:00 am - 5:00 pm.
