Gellert Health has redesigned healthcare for the most vulnerable and underserved populations through the delivery of Health Navigation Services with the goal of improving the patient’s physical and behavioral health outcomes. We prioritize the two greatest barriers to quality healthcare.
- Supporting our patients as they learn & navigate the healthcare system
- Identifying safe and reliable transportation
JOB SUMMARY
Scope of Work:
The Navigator is classified as a BEHAVIORAL HEALTH PARAPROFESSIONAL (BHPP). As specified in A.A.C. R9-10-101, a BHPP is an individual who is not a behavioral health professional who provides health services at or for a health care institution according to the health care institutions policies and procedures that: 1. If the behavioral health services at or for a health care institution according to the health care institutions policies and procedures that: 1. If the behavioral health services were provided in a setting other than a licensed health care institution, the individual would be required to be licensed as a behavioral professional under A.R.S. Title 32, Chapter 33; and 2. Are provided under supervision by a behavioral health professional.
For the provision of family and peer support services, a copy of the peer support employment
training program certificate is required per AMPM Policy 963 and AMPM Policy 964.
DIRECT NAVIGATION OF PATIENT PANEL
The Health Navigator will have a panel of vulnerable patients with an expectation of:
- Schedule patient appointments
- Arrange transportation for or drive patients to and from their appointments in their own car
- Ensure patients receive their prescriptions
- Provide Peer Support services
- Document services provided in the EMR in a clear and concise manner.
- Fulfill additional responsibilities as assigned by Direct Supervisor
Assessment of Navigation skills and competency:
- Weekly individual supervision
- Ongoing monitoring of job performance
- Quarterly Review & Annual Evaluation
Pay and Benefits:

- 2 weeks PTO – increases to 3 weeks in third year of employment

Required prior to employment:
- AZ Fingerprint Card
- CPR/First Aid certification
- COVID-19 Vaccine
- TB Test/Flu Shot
PROBATIONARY PERIOD
The first 90 days of new employment or when accepting a new position shall be a probationary period and your employment may be terminated during this period at any time without notice during your first month, and with one week’s prior notice thereafter. A Gellert Health employee who accepts a new position in the company and fails at securing the new position may be offered a return to their previous position when available.
GH may, at our discretion, extend this period for a further 90 days. During this probationary period your performance and suitability for continued employment will be monitored.
New hires receive the following during the probationary period:
- Extensive training and support from their supervisor
- Weekly check-ins with the supervisor to receive feedback
- Support for improving performance to meet our standards and expectations
- A written performance evaluation at the end of the probationary period
Upon successful completion of the probationary period, the employee is granted permanent employee status. Completing this probationary period does not guarantee continued employment with Gellert Health. Employment is at-will and can be terminated at any time with or without cause. The employee is subject to all company policies and procedures.

To keep our team members and patients safe, we provide PPE, have a strict set of guidelines that you must follow, and recommend regular COVID-19 testing. The Health Navigator is a “Safety-sensitive” position that Gellert Health in good faith believes could affect the safety or health of the employee performing the task or others.

REQUIREMENTS:
- At least 21 years old
- 39 months clear Motor Vehicle Record
- Auto insurance.
- Ability to successfully pass a criminal background check. (Any convictions will be evaluated based upon the conviction in relation to the job requirements. A criminal conviction is not necessarily a deterrent to employment.)
- Associate degree or bachelor’s degree, or work experience equivalent
- Experience working with culturally diverse, chronically homeless, and vulnerable individuals, in medical and behavioral health services (preferred).
- Clean and working four-door car
- Current Arizona driver’s license
- Bilingual – Spanish highly desirable not required.
