THE POSITION:

The Patient Problem-Solving Specialist is an interval piece of coordination efforts within SPA 2 Coordinated Entry System (CES); This position will serve as a linkage between hospitals and homeless service providers, fostering relationships between organizations and facilitating transitions of care for people experiencing homelessness. This position will assess and refer high utilizers to ensure a service connection is made to an appropriate CES Access Site or Outreach Team. In addition, this position will work with internal departments and community partners to ensure that reconnections are made when necessary. The Patient Problem-Solving Specialist will provide technical assistance to hospital staff while ensuring that ongoing training and education on CES resources is provided.

WHAT YOU'LL DO:

  • Cultivates strong, collaborative relationships with hospital partners.
  • Trains and educates hospital staff on access to resources within CES as well as other relevant resources for people experiencing homelessness.
  • Serves as a liaison between hospitals and community providers.
  • Participates in hospital case conferencing and collaborates in problem-solving efforts for people experiencing homelessness.
  • Provides problem solving interventions to people experiencing homelessness.
  • Provides ‘on-call’ technical support to hospital staff as necessary.
  • Assists in tracking participant’s homeless service connection and re-connecting to providers as necessary.
  • Triages high acuity frequent hospital-utilizers and connect to CES staff as appropriate.
  • Refers patients experiencing homelessness to CES Access Sites for assessment and service connection.
  • Provides updates to homeless service providers who have participants entering hospitals
  • Tracks metrics as assigned by supervisor.
  • Documents all participant interactions through case notes using clinical documentation and HIPAA standards, and maintains timely, thorough, and accurate records for reporting purposes in HMIS database.
  • Encourages hospital participation in appropriate CES collaborative meetings.
  • Additional tasks, projects and responsibilities as assigned by supervisor.

WHAT YOU'RE SKILLED AT:

  • Knowledge of homeless populations, systems and programs, specifically the Coordinated Entry System (CES)
  • Knowledge of how hospital systems and CES interrelate and impact one another
  • A thorough understanding of community resources available to individuals experiencing homelessness and how to access those resources
  • Ability to work independently in the field and as part of a team in various settings as necessary
  • Ability to build relationships with stakeholders
  • Ability to present information thoroughly to a range of stakeholders
  • Ability to demonstrate creative and solution-focused problem-solving skills
  • Proficient in Microsoft Programs (Word, Excel, Access, PowerPoint), Zoom, and web-based software

OTHER:

  • Maintain and execute confidential information according to HIPAA standards
  • Obtain and maintain CPR/First Aid Certification
  • Ability to pass post-offer Tuberculosis (TB) clearances
  • Travel is a regular duty for this position and is required 50% of the time
  • Must have and main a valid California Driver’s License and auto insurance in good standing
  • Use of a personal vehicle to travel between worksites and other locations is required
  • Ability to wok a 9/80 work schedule

WHAT YOU'VE ACCOMPLISHED:

  • At least two (2) years’ experience in a system coordination role preferred
  • At least two (2) years’ experience in a hospital setting, medical setting, or health care patient advocacy role preferred
