ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Manage a Caseload of Diverse Individuals
  • Provide each individual with the opportunity to live his/her own Everyday Life
  • Develop a rapport with individuals’ families and county representatives to facilitate communication in all aspects of individuals’ lives
  • Attend team meetings as necessary to provide input on an individual’s progress during program
  • Schedule and attend doctor’s appointments as needed with individuals
  • Perform satisfaction surveys on a quarterly basis to ensure individuals are happy with their service
  • Reinforce that individuals’ outcomes are being met during program, and that they are appropriate
  • Provide direct service as needed to ensure quality of program
  • Search for and provide opportunities for individuals to advance their lives
  • Provide emotional support to individuals through difficult life situations
  • Assist in identifying individuals’ strengths and how they can use them in the community

  • Supported Employment
  • Adhere to the “Provider Agreement” between OVR and The Arc; oversee supported employment services as defined within the agreement
  • Provide individuals with frequent and intensive support prior to employment, during initial employment and after the provision of OVR services to maintain employment
  • Develop Supported Employment Support Plans/Goals
  • Assist individuals to enter competitive integrated employment as quickly as possible, achieve stability in a competitive integrated work setting with natural supports, and move into extended services, as needed with other long-term funding sources
  • Develop a fading plan for services as appropriate to ensure successful stabilization in employment
  • Attend initial/ongoing OVR meetings as necessary to provide input on an individual’s progress
  • Keep continued line of communication open with OVR counselors and update on all changes
  • Participate in trainings determined necessary by OVR
  • Provide collaboration/support for employers to ensure successful placement in employment locations
  • Submit all service reports through the Commonwealth Workforce Development System (CWDS); OVR may refuse payment for services that have not been properly documented or for incomplete or substandard services
  • Accept or deny referral for SE services within CWDS
  • Submit invoices and documentation of services to CWDS within 30 days of completion of services or phases
  • Maintain updated and accurate information and hours worked in CWDS, making changes no later than 10 days
  • Develop mandatory trainings necessary for job coaches to excel at their position
  • Add new referral information into the appropriate software systems
  • Track Supported Employment Professionals’ trainings
  • Collaborate with scheduling to pair new referrals with Supported Employment Professionals
  • Adjust reports from Supported Employment Professionals to ensure correct information after submission
  • Stay up to date on new services provided through OVR and offer new options to individuals in the program

  • Supervise Supported Employment Professionals & Direct Support Professionals
  • Develop a strong, professional rapport with DSPs
  • Create a welcoming environment for DSPs to thrive
  • Identify strengths of DSPs and assist them in applying these strengths to their daily work
  • Train new and existing DSPs on the specific needs of each individual on your caseload
  • Communicate necessary information to DSPs regarding the individuals they serve
  • Monitor DSPs during work hours to assess their work performance
  • Mentor DSPs having difficulties in work performance by modeling what they could improve
  • Provide critical, strengths-based employee evaluations to assess DSP performance
  • Facilitate positive change in DSPs by providing verbal and written corrections when behavior goes against agency policy
  • Mentor DSPs obtain The Association of Community Rehabilitation Educators (ACRE) Basic Employment Certificate

  • Office Duties
  • Update ISPs and individuals’ records as changes are made
  • Organize and store individuals’ files securely
  • Collect daily progress reports from DSPs and assess them to make sure outcomes are met
  • Approve mileage and business expenses of DSPs
  • Verify employees’ timesheets on a weekly basis
  • Provide documentation quarterly to track each individuals’ progress
  • Attend staff, program specialist, and DSP meetings for agency updates and to provide input on changes going on in the agency
  • Report incidents to HCSIS in a timely manner and follow-up when appropriate
  • Assist as-needed with scheduling to ensure all individuals’ needs for program are met

  • Promotes agency values during and outside of office hours
  • Participates in a wide variety of special projects and committees to improve program quality and diversity for individuals and staff
  • Perform extra-curricular activities as assigned and volunteered for to better the agency
  • Engages in The Arc’s community in a positive and productive way
  • Communicates with co-workers, management, clients, and others in a professional manner
  • Conforms and abides by all regulations, policies, work procedures, and instructions
  • Develop a basic understanding of all programs The Arc offers to individuals receiving services to communicate as needed
  • Collaborate with potential employers to secure integrated employment locations for individuals
  • All other duties as assigned.

EDUCATION AND/OR EXPERIENCE: A bachelor’s degree in Human Services or a related field is preferred but not required. A high school diploma is required.
