Qualifications:

  • Licensed Social Worker
  • Two plus years of experience working in a skilled nursing facility
  • Ability to multi task duties
  • Must have knowledge of Dept. of Public Health regulations
  • Have a working knowledge of completing PASSR
  • Understands Advanced Directives requirements.

Duties Include

  • Coordinates interdisciplinary team care plan meeting
  • Facilitates discharge planning for short term stay residents with family.
  • Knowledge of community resources and arranges support services for residents safe discharges.
  • Liaison for family members
  • As a department head participation in quality improvement meeting is required and necessary to report department orperating satus.
  • Oversees and directs assistants social worker.
