Are you ready to take on a dynamic leadership role? We are seeking a passionate individual to oversee our Home-based department and make a real difference in the lives of our clients. As the Home-Based Program Coordinator, you will be responsible for supervising clinical services, ensuring program effectiveness, and providing exceptional care to our clients.

MUST BE CLINICALLY LISCENCED: Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC) or Licensed Marriage and Family Therapist (LMFT) by the State of Indiana

Offer Details:

  • Starting Pay: $80,000+ (Depending on Experience)
  • Sign On Bonus $3,000

Duties and Responsibilities:

Supervisory Functions: Lead the coordination and oversight of clinical services, including providing onsite supervision, ensuring staff compliance with standards, and addressing customer concerns. You will play a crucial role in achieving program targets and ensuring exceptional patient experiences.

Client/Group Services: Provide effective mental health interventions and crisis interventions in various settings, including homes, schools, and community-based sites. Collaborate with multidisciplinary teams to develop comprehensive treatment plans and discharge plans tailored to each client's needs.

Care Plan: Work closely with treatment teams to facilitate treatment planning and coordinate services. Conduct psychosocial assessments, develop individualized treatment plans, and link clients to community resources and services.

Department Accountability: Adhere to all organizational policies and procedures, including participation in aggressive behavior management training. Ensure compliance with behavioral health services policies and maintain accountability for departmental operations.

Qualifications:

- Master’s Degree in a mental health-related field

- Three years of relevant clinical experience in mental health/behavioral health

- One year of administrative leadership experience

- Valid Indiana license (LCSW, LMHC, LMFT) and current Indiana Driver's License

- Strong interpersonal skills, ability to manage time and priorities independently, and effective communication skills
