Position: Program Coordinator

Reports to: Program Manager

Status: Full-time, Salary Exempt

Location: Indianapolis, IN

Education: Bachelor's Preferred, not required. Experience Required.

Typical Schedule: Monday through Friday (Requires being available for on call emergencies/supports-including weekends for assigned homes), Rotating Weekend On-Call

Position Summary: Program Coordinator is responsible for the oversight and coordination of services provided to Community Integration Support Services (CISS) clients. The Program Coordinator supervises Direct Care Professionals (DSP) and provides training, mentoring, coaching and positive supervision to staff on an on-going basis. Specifically, the Program Coordinator is responsible for ensuring DSPs are fully and appropriately trained, documentation is current and accurate, services provided are in full compliance with all state and federal laws, the consumer’s PCISP, BSP and other applicable plans.

The Program Coordinator will have a specific caseload, specialized assignments and are assigned to the North, East, South, West and Central areas within the state of Indiana.

Responsibilities/Duties: The following job responsibilities/duties are general in nature and should be considered guidelines, and not a definitive list. Program Coordinators are asked to work together in a team environment and, as a result, may be asked to assume additional responsibilities deemed necessary that are not listed herein.

  • Direct, manage and administer the coordination and service delivery components of programs that service consumers in residential settings
  • Supervise the delivery of service and ensure individuals and consumers are receiving the highest quality care
  • Routinely observe service delivery on-site and monitor for demonstration of knowledge of consumer health needs, behavior management techniques, and emergency procedures.
  • Serve as a liaison between the community and agencies in the service delivery system, family/guardians, and the agency
  • Communicate/collaborate with other providers, families, and consumers.
  • Report, identify and resolve conflicts between staff members, clients, and client family members and/or guardians
  • Ability to be on call for emergencies for the homes assigned on their caseload
  • Complete IRs
  • Train, evaluate, and monitor assigned DSP’s
  • Complete Performance Evaluations to ensure employees are productive, accountable, and successful in their positions
  • Conduct regular in-home visits
  • Complete quarterly reports and other reports
  • Review and approve staff time
  • Complete schedules
  • Participate in care coordination
  • Attend consumer’s quarterly and annual meetings, and other meetings, as necessary
  • Make recommendations regarding promotions, compensation, bonuses, staff recruitment, retention, and disciplinary actions
  • Participate in transitions
  • Complete Risks Assessments/Plans

Experience and Qualifications:

  • High school diploma or equivalence
  • Minimum of three (3) years of experience in the Developmental Disability field, with at least one (1) year spent providing Direct Care Services
  • (1) year of supervision
  • Must meet all personnel requirements as set forth in 460 IAC 6
  • Valid U.S. Residency

Work Environment:

  • Position requires frequent travel and work in residential settings, meeting spaces and company office.

Physical Requirements:

  • Ability to walk, sit, stand, squat, bend, reach, stretch, lift, push, and pull
  • Ability to sit for extended periods of time in front of a computer or for purposes of travel

Job Type: Full-time

Pay: Starting at $42,500 per year
