Responsible to: Personal Care/Wellness Administrator

Purpose of your Job Position

The primary purpose of your job description is to assist in the administration of medications, treatments, and the assessment of health care needs of independent residents who reside in Riddle Village. The administration of medications shall be in accordance with established nursing standards, the policies, procedures, and practices of this facility and the requirement of this state.

Delegation of Authority

As a Wellness Center Licensed Professional Nurse, you have delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. In the absence of the Personal Care/Wellness Administrator, you are charged with carrying out the resident care policies established by the facility.

Duties and Responsibilities

Assess the health care needs of independent residents in the Riddle Village Community.

Accurately and safely prepare, administer and document medication administration for residents requiring such assistance. This includes: ordering medications, faxing monthly updated medication orders to the primary physicians, adjusting medications as ordered, checking and verifying medications in the card with corresponding physician orders, updating and signing off the medication cardexes, distributing medication packages weekly to residents who understand the correct dose, times and have knowledge of their medication, and ensuring residents unable to manage medication packages have the option to receive their medications at the Wellness Center (or the PCU Unit after 4:00 p.m.).

Administering annual and pre-employment tuberculin tests, using the intradermal injection method.

Assess and initial treatment of any employee injury. Determine appropriate follow-up and assist with plans.

Health teaching including pre-operative, post-operative, diabetic teaching accu-check use, oxygen equipment, medications, disease process, etc.

Responding to priority calls within a time frame of five minutes.

Visit any resident who fails to check in daily using the electronic system.

Provide follow-up visits or calls to a discharged resident for three (3) days minimum.

Provide follow-up visits/evaluations after a fall or illness, or to any resident requiring medical care. Ensure adequate medical treatment and assessment are provided to meet individual needs.

Perform monthly checks on equipment in the Wellness department and the community defibrillators.

Evaluation and assessment of residents discharged from the hospital or Monticello House. This includes: assisting with transfer, reading and explaining discharge instructions with the resident, checking the current medications found in the apartment with prescriptions on the discharge orders, assessing the individual's ability to manage independently, order homebound meals for a maximum of 14 days, and updating calls to family and doctor.

Monitor and record vital signs as necessary.

Assess, evaluate and re-dress wounds.

Organize and assist in the administration of yearly influenza vaccines for independent residents and employees.

Perform administrative requirements, such as, completing necessary forms, incident reports, charts, reports, daily communication reports and submission in a timely manner.

Perform pre-admission interviews and evaluations including medical, psychological, and mental capacity. Prepare a report for Medical Director of the interview and evaluation.

Maintain a friendly, productive, working relationship with Riddle Village employees, attending physicians, pharmacist, guests, and residents.

Ensure resident rights are followed at all times.

Provide a daily report to the Personal Care Unit at day's end prior to leaving the building.

Documentation is informational, descriptive and includes the resident's response to treatment.

Assist in the development, maintenance and periodic updating of written policies and procedures that govern the daily functioning of the Wellness department.

Assist with the ongoing Quality Assurance program for the Wellness Office.

Attend educational programs including in-house and professional programs to maintain professional.

Calculate monthly billing charges for fee-for-services, submitting to the resident and the billing department.

Follow established departmental policies and procedures, including dress codes, infection control, safety procedures, fire safety and disaster preparedness drills in a safe and professional manner.

Working Conditions:

Works in an office area, resident rooms, and throughout the community.

Moves intermittently during work hours.

Is subject to frequent interruptions

Is involved with residents, guests, employees, and government agencies.

Subject to hostile and emotionally upset residents, family, personnel, and visitors.

Communicates with medical staff, nursing personnel, and other department supervisors.

Works scheduled days and weekends may be required to work beyond regular hours.

Subject to call back for emergencies: weather, evacuation, disaster, etc

Involved in community/civic programs as required.

Attends and participates in continuing education programs.

Subject to exposure to infectious wastes, diseases, conditions, such as TB, Hepatitis B, MRSA, AIDS.

Maintains a liaison with the residents, families, and support systems, to adequately plan for resident needs.

Educational requirements:

Must possess a current, unencumbered, active license to practice as an LPN in the state of Pennsylvania.

Specific Requirements:

Must possess a current, unencumbered, active license to practice as a professional nurse in this state.

Must be able to read, write, speak, communicate, and understand the English language.

Must possess the ability to make independent decisions when circumstances warrant.

Must possess the ability to deal tactfully with personnel, residents, families, guests and governmental agencies, with a cheerful disposition and enthusiasm, as well as a willingness to handle difficult situations.

Must be knowledgeable of nursing and medical practices, procedures, laws, regulations, and guidelines that pertain to Long-Term Care.

Must possess leadership and supervisory skills and the willingness to work harmoniously with others.

Must have the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.

Must seek out new methods and principles and incorporate them into existing nursing practices.

Must be able to relate information concerning a resident's condition.

Must not pose a direct threat to the health or safety of other individuals in the workplace.

Physical and Sensory requirements:

Must be able to move intermittently throughout the workday.

Must be able to cope with the mental and emotional stress of the position.

Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.

Must meet general health requirements, set forth by the policies of this facility, which include a medical and physical examination.

Must be able to lift a minimum of twenty-five (25) pounds and push, pull and move resident's equipment supplies, etc. up to fifty (50) pounds.

May be necessary to assist in the evacuation of residents during emergency situations.
