Summary: The Licensed Practical Nurse (LPN) will provide health services and direct care to individuals with disabilities living in a residential program. In addition the incumbent with provide advocacy and liaison with community health care providers, monitoring medical records, and medication administration.

Schedule: Monday - Friday 8:30 AM - 3:30 PM (35 hours)

Essential Duties and Responsibilities Include:

  1. Provide direct specialized nursing and other day-to-day care to individuals.
  2. Document health care services following appropriate state regulations, nursing standards, and Massachusetts LPN license requirements. Abide by all agency policies and procedures.
  3. Function under the supervision of the residential manager, in collaboration with the registered nurse and Director of Health Care.
  4. Act as a liaison as needed to other community agencies and treatment providers regarding regular medical needs and care. Help staff identify appropriate community health resources.
  5. Provide in-service education to individuals and/or staff on health issues.
  6. Maintain knowledge of the Department of Public Health Medication Administration Program (MAP) current policies and procedures.
  7. Administer medications to assigned individuals following MAP policies and procedures
  8. Provide quality assurance reviews of medication systems and MAP compliance.
  9. Work collaboratively with the residential manager and registered nurse evidenced by meeting weekly or as needed for medical updates.
  10. Work collaboratively with the nursing/healthcare staff of Bridgewell by participating in monthly nursing staff meetings and agency-wide healthcare planning.
  11. Collaborate with the residential manager and registered nurse to assess, organize, update, and maintain health care and medication needs. This should include, but is not limited to:
  • Individual client assessment
  • Review of routine and other physical exams
  • Review of physician-ordered treatments
  • Assurance of treatment follow-up and coordination of medical and psychiatric care
  • Maintenance of medical charts.
  • Other duties as assigned

In Addition, your Direct Support Responsibilities Include:

  1. Provide support and assist individuals with all activities of daily living which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements.
  2. Provide care to individuals such as checking vital signs, checking their weight, applying creams/ointments, etc.
  3. Maintain the comfort, privacy, and dignity of each individual in the delivery of services to them.
  4. Observe residents for changes in condition or behavior and promptly report these changes to the Residential Manager.
  5. Provide services to individuals according to DDS, CARF, and all applicable regulatory standards.
  6. Provide support and assist individuals with all medical, clinical, and financial needs by identifying needs and securing appropriate services as needed.
  7. Transport individuals as needed to and from medical, recreational, and other activities outside the home.
  8. Assist with housekeeping and cleaning duties as needed to maintain a safe and clean living environment.
  9. Maintain progress notes according to DDS and all applicable regulatory standards.
  10. Write annual assessments on individual goals as outlined in the Individual Service Plan (ISP) and participate in the ISP meeting and other relevant meetings for the individuals.
  11. Assist the individuals with accessing resources and recreational opportunities in the community.
  12. Comply with Bridgewell Awake Overnight Call-in Procedures if applicable to assigned shift.
  13. Maintain core training certifications including CPR/FA, OSHA, and PABC, as well as completing training in MAP.

Required Education & Experience:

  1. Graduation from an accredited and approved licensed practical nurse program.
  2. Must have current Massachusetts Nursing license.
  3. Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting is preferred.
  4. Experience working with adults with DD and behaviors is a plus.
  5. Must have a Driver's license for at least one year with a good driving record history.
  6. Must be able to drive a 12-passenger van. We will provide training.

Required Skills/Knowledge:

  1. Excellent written and oral communication skills.
  2. Excellent organizational and problem-solving skills.
  3. Ability to foster positive relationships.
  4. Must have basic to intermediate computer-level competency.
