Knowledge and skills required for the position are:
- RN provides initial and ongoing assessments to identify the client's medical needs
- level of functioning and ability to communicate and learn.
- RN initiates the plan of care
- necessary revisions
- and performs OASIS assessments at appropriate time points when applicable.
- RN Initiates and performs appropriate nursing interventions.
- RN coordinates services
- informs the physician and other personnel of changes in the client's condition and needs.
- RN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
- RN Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
