ABOUT THE ROLE  

At Hopscotch Primary Care, we take a team approach to serve patient needs and provide the best care possible.  Our goal is to provide the care each of us would want for ourselves or for our family members, in the right setting, and at the right time.  The Hopscotch Care Team is responsible for delivering high-touch, high-quality care to patients, including coordination of care across different physicians and the broader healthcare continuum.   

As Associate, Population Health, you will play a critical role in designing and supporting implementation of our population health operating model. The role will support core population health domains including but not limited to new program development, workflow optimizations, assessing vendor support and other tools, and driving excellence across all domains. This role will report to the Population Health Director.

WHAT YOU'LL DO

Core Responsibilities:  

  • Collaborate closely with a cross-functional team to build out population health capabilities that contribute to a best-in-class Hopscotch care model for rural populations
  • Work with population health team and technology team to scope and execute new initiatives through internal and external research, vendor conversations, development of business cases and workflow development
  • Assess, refine and develop workflow optimizations to enable best-in-class clinical outcomes for the Hopscotch model
  • Analyze data using excel and other tools to create insights to build new programs and drive operational efficiencies
  • Actively participate in program rollout by spending time with clinic teams and auditing performance
  • Develop solutions that scale across multiple locations

ABOUT YOU  

We are looking for a motivated, mission driven, and experienced associate with: 

  • 1+ years of relevant experience in consulting, healthcare, population health, change management, or other similar experience with strong preference for value-based care models
  • Experience working with excel and powerpoint, other data platforms and electronic health records
  • Strong organizational skills and the ability to manage multiple priorities appropriately
  • A growth mindset with a willingness to learn complex topics quickly and execute effectively
  • US work authorization
  • Bachelor's Degree required
