About the Role

The Hopscotch Care Team is responsible for delivering high-touch, high-quality care to patients, including coordination of care across different physicians and the broader healthcare continuum.

As a key member of our team, you will play a critical role in designing and supporting implementation of our population health operating model. This will involve collaborating closely with a cross-functional team to build out population health capabilities that contribute to a best-in-class Hopscotch care model for rural populations.

Key Responsibilities:

  • Collaborate with the population health team and technology team to scope and execute new initiatives through internal and external research, vendor conversations, development of business cases and workflow development
  • Assess, refine and develop workflow optimizations to enable best-in-class clinical outcomes for the Hopscotch model
  • Analyze data using excel and other tools to create insights to build new programs and drive operational efficiencies
  • Actively participate in program rollout by spending time with clinic teams and auditing performance
  • Develop solutions that scale across multiple locations

About You

We are looking for a motivated, mission-driven, and experienced professional with:

  • 1+ years of relevant experience in consulting, healthcare, population health, change management, or other similar experience with strong preference for value-based care models
  • Experience working with excel and powerpoint, other data platforms and electronic health records
  • Strong organizational skills and the ability to manage multiple priorities appropriately
  • A growth mindset with a willingness to learn complex topics quickly and execute effectively
  • US work authorization
  • Bachelor's Degree required
