About the Role

Howard Brown Health is a nationally recognized leader in LGBTQ+ health and wellness, dedicated to providing inclusive and affirming care to the Chicago community. As a Physician in our community health setting, you will play a vital role in addressing the healthcare needs of diverse populations, including African American/Black community members.

Responsibilities
  • Facilitate patient entry into the healthcare system, assessing primary health care needs and providing comprehensive health histories, physical examinations, and diagnostic tests.
  • Provide coordinated, continuous, and culturally competent primary health care to clients, spanning the health continuum through mutual goal setting and planning delivery of care.
  • Prescribe treatment modalities to meet health and illness needs, collaborating with advanced practice nurses and community health nurses to prevent duplication of services.
  • Meet average monthly provider productivity standards, including number of visits, and adhere to Patient-Centered Medical Home Standards.
  • Manage and evaluate the plan of care, process, and outcome, modifying the plan as needed, and adhere to collaborative practice principles of respect and joint decision making.
Requirements
  • Graduation from a college or university accredited by the American Medical Association with a Doctor of Medicine degree or accredited by the American Osteopathic Association with a Doctor of Osteopathy degree.
  • Successful completion of an approved program of residency and experience in establishing and maintaining effective working relationships with other employees, patients, and the general public.
  • Knowledge of the principles and practice of medicine and the structure and function of community health centers.
  • Ability to read, analyze, and interpret complex documents, communicate effectively with patients and their families, and work with mathematical concepts such as probability and statistical inference.
