Duties & Responsibilities

  • Provides clinical case management and counseling services to clients and their families deemed level 3 post-release services cases.
  • Receives case assignments, establishes treatment plans, reviews case progress, and determines case closure with the support of the clinical supervisor of the assigned program.
  • Meets regularly with assigned cases at in-home visits.
  • Establishes and maintains therapeutic and professional relationships with clients to facilitate the receipt of services.
  • Assesses, plans and supplements the client’s level of functioning.
  • Develops service plans, which outline a specific plan of action with each client under the regular supervision of the clinical supervisor of the assigned program. Develops, implements, oversees, and follows up with client treatment plans to ensure that realistic goals and deadlines of accomplishments are being met.
  • Maintains documentation of the client’s treatment.
  • Provides individual counseling to address motivational and psychosocial problems of unaccompanied children.
  • Provide intense advocacy, crisis intervention, and problem-solving in a variety of areas for all clients, as appropriate.
  • Provides clients with concrete assistance and resource connection to support the client’s capacity to heal from trauma and enhance safety and stability. Makes referrals by service plans to other Organization units and external providers.
  • Uses the knowledge of the public assistance agency requirements for benefits and entitlements to advocate on behalf of clients.
  • Develops, monitors, and enhances professional relationships with social service, health, and governmental agencies.
  • Participates and contributes in regular supervision meetings, team meetings, in-house training sessions, conferences, and program and operational planning.
  • Accurately completes promptly all necessary forms, case recordings, and statistical reports.
  • Establishes and maintains a professional and therapeutic relationship within the standards of client profiles, confidentiality laws, and the newest clinical research available.
  • Maintains the volume of work designated by project standards and utilizes the mental health theory and concepts integrated into clinical practice. On an ongoing basis assimilates the knowledge of resources available to implement the clinical concepts.
  • Assumes all clinical responsibility for clients.
  • Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), private funding sources, Organization plans/policies, and Unit guidelines. Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to such standards. Initiates requests for assistance from the Supervisor to address new issues or complex concepts affecting adherence to professional standards.
  • Other duties may be assigned.

Education and Experience:

  • Master’s degree (M.A.) in clinical social work, counseling or clinical psychology;
  • Minimum two (2) years of clinical practice experience in either a hospital or community setting providing group and/or individual therapy

Certifications/Licenses:

  • Eligible to become a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC.)

Knowledge/Skills/Abilities (K/S/A):

  • Ability to speak and be fluent in a second language including but not limited to the following multi-language abilities:
    • Ability to read and interpret documents such as operating and maintenance instructions, and procedure manuals.
    • Ability to write routine reports and correspondence.
    • Ability to speak effectively before groups of customers or employees of the organization.
  • Access to regular, dependable, and timely transportation
  • Ability and comfortability working with people of varying socioeconomic backgrounds.
  • Excellent communication skills. Strong networker, convener, relationship builder, and communicator with experience collaborating with diverse work teams and stakeholders.
  • Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations professionally and diplomatically.
  • A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability.
  • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to work independently and remain self-motivated with minimal direct supervision.
  • Strategic thought partnership and consultancy skills.

Work Arrangement: This role is eligible for a “hybrid” work arrangement. It is expected this person would be available for in-home visits for level 3 PRS cases.

Work Environment:

  • The noise level in the work environment is usually moderate.
  • These residential programs require that employees are available to work any shift, including evenings and weekends, according to programmatic needs, licensing requirements, and/or contract requirements.

Physical Demands:

  • While performing the duties of this job, the employee is regularly required to talk and hear.
  • The employee is regularly required to sit, stand, and walk.
  • The employee must be able to stoop, kneel, and/or crawl.
  • The employee is regularly required to use hands to key, handle, or feel and reach with hands and arms.
  • The employee must occasionally lift and/or move up to 25 pounds.
  • The employee must have unrestricted ability to provide physical restraint.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
