We are seeking a highly skilled and compassionate Mental Health Clinical Practitioner to join our team at Heartland Human Care Services. As a key member of our Children's Programs, you will play a vital role in providing short-term case management, counseling, and support to empower children and families.
Key Responsibilities- Short-Term Case Management: Develop and implement treatment plans to address the unique needs of each child and family.
- Counseling and Support: Provide individual and group counseling, as well as crisis intervention and creative problem-solving to address motivational and psychosocial challenges.
- Therapeutic Relationships: Cultivate strong, empathetic relationships with children and families, creating a safe and nurturing environment for growth and development.
- Collaboration and Communication: Work closely with specialists, agencies, and other stakeholders to ensure seamless coordination of care and maximize positive outcomes.
- Documentation and Record-Keeping: Maintain accurate and detailed records of client interactions, treatment plans, and progress.
- Education and Experience: Master's degree in clinical social work, counseling, or related field, with 1-2 years of clinical practice experience in a hospital or community setting.
- Licensure and Certification: LPC, LSW, or relevant certification/licensure required (or obtained within 1 year).
- Language Skills: Bilingual proficiency in a second language (both written and oral) is highly desirable.
Our residential programs require flexibility and adaptability, with the ability to work various shifts, including evenings and weekends, according to programmatic needs and licensing requirements.
