Shift: On-Site, Sunday to Thursday (12:00pm to 8:30pm)


Summary:


Under the general guidance of the Clinical Supervisor, the Mental Health Clinical Practitioner provides direct clinical case management and counseling services within the Opportunity Youth Programs. This position services Juvenile Justice and/or DCFS involved youth, ages 12 – 18, who reside in shelter/residential programs. This participant population is affected by trauma, may experience mental health concerns, may display significant behavioral concerns, and may require crisis intervention. As this position is within a residential program, the employee may be required to remain on Organization premises during break and/or meal periods.

Essential Functions:

Duties & Responsibilities

  • Provides services to clients through a short-term case management model. Includes receiving case assignments, managing intakes, and completion of treatment plans, individual and group counseling and notes, individual assessments, and case closures.
  • Meets regularly with assigned cases through outreach to shelters and communities at large and provides individual, group counseling, and crisis stabilization interventions, to address the motivational and psychosocial problems of participants and their families.
  • Provides clients with counseling and concrete assistance to support increase or restore client’s capacity for self-sufficiency and links participants to needed community resources, including benefits and entitlements, or financial supports.
  • Establishes and maintains therapeutic and professional relationships with clients to facilitate the receipt of services in accordance with professional ethical guidelines and Organization policies, confidentiality laws and the newest clinical research available.
  • Develops, monitors, and enhances professional relationships with social service, health, and governmental agencies.
  • Participates and contributes to regular supervision meetings, team meetings, in house training sessions and conferences, and participates in program and operational planning.
  • Accurately completes in a timely manner all necessary forms, case recordings and statistical reports.
  • Develops, implements, and reviews psychoeducational, social, or therapeutic groups for participants of the assigned program, in collaboration with supervisor of the assigned group.
  • Provides intense advocacy, crisis intervention, counseling and problem solving as needed for participants.
  • Assumes all clinical responsibility for clients and manages or participates in various project meetings, multidisciplinary staffing, or case reviews, and maintains documentation of these meetings.
  • Develops and maintains relationships with outside providers, case managers and hospitals.
  • Attends a minimum of 15 educational trainings per fiscal year.
  • Assists with psychiatric services provided within the program, such as coordinating with the psychiatric provider, supporting medication administration as appropriate, as specified by the Psychiatrist and keeps legible and accurate records of all medication issues and consulting with the Psychiatrist regarding treatment reviews.
  • Provides transportation of program participants as needed.
  • Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), private funding sources, Organization plans/policies and Unit guidelines. Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to such standards. Initiates requests for assistance from Supervisor to address new issues or complex concepts affecting adherence to professional standards.
  • Other duties may be assigned.

Education and Experience:

  • Master’s degree (M.A.) in clinical social work, counseling, or clinical psychology.
  • Minimum two (2) years of clinical practice experience in either a hospital or community setting providing group and/or individual therapy.

Certifications/Licenses:

  • Eligible to become a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC.)

Knowledge/Skills/Abilities (K/S/A):

  • Written and oral language abilities are required as follows:
  • Ability to read, draft or interpret documents such as procedure manuals, case note and psychosocial summaries, service, and treatment planning, and other. 
  • Ability to write routine reports and correspondence. 
  • Ability to speak effectively before groups of customers or employees of organization.
  • Access to regular, dependable, and timely transportation
  • Ability and comfortability working with people of varying socioeconomic backgrounds.
  • Excellent communication skills. Strong networker, convener, relationship builder and communicator with experience collaborating with diverse work teams and stakeholders.
  • Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations in a professional and diplomatic manner.
  • A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability.
  • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Human Care Services.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to work independently and remain self-motivated with minimal direct supervision.
  • Strategic thought partnership and consultancy skills.

Work Arrangement:


This role is not eligible for remote work. All hours will be worked in person. This position has an active presence at the assigned program.

Work Environment:

  • The noise level in the work environment is usually moderate.
  • Each program site operates on a 24/7 basis, 365 days per year. To meet both the needs of our participants and contractual obligations, employees are required to be flexible in regard to scheduling and work location.
  • Program sites are located throughout the Chicago area, including the North and South sides of Chicago, as well as the suburbs. Employees may be required to report to any one of our locations on a temporary or permanent basis.

Physical Demands:

  • While performing the duties of this job, the employee is regularly required to talk and hear.
  • The employee is regularly required to sit, stand, and walk.
  • The employee must be able to stoop, kneel and/or crawl.
  • The employee is regularly required to use hands to key, handle, or feel and reach with hands and arms.
  • The employee must occasionally lift and/or move up to 25 pounds.
  • The employee must have unrestricted ability to provide physical restraint.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
