Basic Function and Scope of Job

Reporting
to the GSSC Manager, the Geriatric Support Services Coordinator/Supervisor is
responsible for coordinating support services as part of the Senior Care
Options Primary Care Team in compliance with regulations set forth by the
Senior Care Organization and Elder Services of Cape Cod and the Islands. The
Geriatric Support Services Coordinator/Supervisor’s basic functions are
responsibility for the orientation, ongoing training and day-to-day supervision
of Geriatric Support Services Coordinators and maintaining a personal caseload to
assist eligible SCO members to obtain services which will enhance their quality
of life

Work
Performed and Judgment Required

As
Supervisor conduct clinical consultations with GSSC’s, monitor, review and
evaluate their work performance.

Specific
tasks include as supervisor and caseload maintenance:

  • Perform initial and ongoing
    assessments of the health and functional

status of the member.

  • Develops a community based care
    plan and related service package.

Submits plan to the SCO team
members.

  • Assist in the development of the
    member’s Individual Care Plan.
  • Track member transfers from one
    setting to another, work to adjust the

Individual Care Plan as appropriate.

  • Coordinate direct services to
    members.
  • Assist members with a Loss of Mass
    Health.
  • Meets monthly with SCO team members
    to review cases.
  • Maintains case records as required
    by the Agency, Executive office of Elder Affairs and the Senior Care
    Organization.
  • Participates in in-service
    trainings, courses and workshops.
  • Participates in special initiatives
    and projects within the agency.
  • Other duties as assigned.

Accountability

Accountable to the GSSC Manager for all job
responsibilities.

Contacts

Contacts made on a daily basis with the entire
range of Agency personnel

and SCO team. Contacts made on a regular basis
with members of the community, service providers, consumers/caregivers and
their families.

Travel

Travel required daily throughout the service
area. Travel to training and

meetings as needed (on Cape and off Cape).

physical/mental/Visual
demands

High
Computer use daily both in the office and in the field.

Working
Conditions

Typical office
conditions. Frequent use of general office equipment.

Supervisory
Responsibility

Clinical supervision of GSSC’s. Supervisory load is subject
to change at any time.

Primary
Requirements

A Bachelor’s degree in social work or related
human services field;

Massachusetts State licensure in social work
preferred.

Minimum of two years professional experience
with at least one year in a setting where elders receive health care services
such as a hospital, nursing facility a community health agency or
interdisciplinary program within an ASAP.

Supervisory experience preferred

Ability to communicate and work with the
elderly and community resources serving the elderly population.

Ability to work independently and within an
interdisciplinary team.

Excellent computer skills required.

Valid MA driver’s license and car required.
