Direct Primary Care practice in Blue Ash , Ohio is searching for a Behavioral Health Therapist to complete our clinical team.

DPC is an innovative way of serving patients that we are seeking to improve further by offering in-house behavioral health resources. Operating as a practice expert in behavioral health. The therapist will function as a critical component of the healthcare team. They also assist in wholistic care of the patient by identifying, triaging, diagnosing, treating and referring patients presenting with mental specific health needs during their primary care visit. The therapist serves patients and support to the health care team by providing skills training through psychoeducation, problem solving behavior change needed to manage chronic disease states, teaching stress management as well as reflection techniques as well as proactively addressing behavioral health needs to optimize patient wellness.

Our practice is developing a preventative approach to mental wellness and is looking for an individual with strong clinical skills in range of therapy modalities, who is also interested in developing and furthering the field of integrated mental healthcare. The ideal candidate will have experience in primary care or medical setting providing integrated care. They would also have an interest in innovation and program development.

Responsibilities:

  • Actively participate with Health care team
  • Conduct behavioral health screenings and assessments
  • Develop and run treatment groups
  • Develop psychoeducation/information materials
  • Experience working with all ages, adults and children in a behavioral health setting
  • Provide counseling and brief interventions
  • Offer behavior change for improving management of chronic disease care such as smoking cessation, diet, exercise, sleep hygiene, stress relief, medication adherence
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Communicate with collaborating physician or specialist regarding patient care
  • Enter adequate health care records
  • Maintain registries to follow patient improvements
  • Other responsibilities as needed

Qualifications:

  • Masters Degree in Social Work or Clinical Mental Health Counseling
  • Licensed LISW or LPCC
  • Knowledgeable about regions mental health and substance abuse treatment system and resources
  • 3 + years experience in BehavialHealth services
