Job Title: Primary Care Nurse Practitioner -  (Mobile/Home Services)

Location: Arizona (Statewide)

Job Type: Flexible Schedule (Part-Time)

Position Overview: We are seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our team. This role involves delivering primary care services to patients in their homes or other mobile settings. You will have the autonomy to manage your schedule based on your availability and patient needs, providing a unique opportunity to balance work and life.

Key Responsibilities:

  • Conduct comprehensive patient assessments, including history, physical examination, and diagnostics.

  • Develop and implement personalized care plans for patients with a variety of medical conditions.

  • Prescribe medications and treatments as appropriate.

  • Collaborate with a multidisciplinary team to ensure the highest standard of care.

  • Provide education and counseling to patients and their families regarding health conditions and preventive care.

  • Maintain accurate and timely patient records.

Qualifications:

  • Active Nurse Practitioner (NP) license in the state of Arizona.

  • Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).

  • Minimum of 2 years of experience in primary care, internal medicine, or a related field.

  • Experience in home health or mobile healthcare settings is preferred.

  • Strong interpersonal and communication skills.

  • Ability to work independently and manage a flexible schedule.
