Job SummaryWe are looking for a Home Care Aide to join our team You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus. 
Responsibilities 
  1. Personal Care: 
    1. Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, medication administration, and toileting. 
  2. Support and Supervision: 
    1. Provide companionship and emotional support to clients. 
    2. Support clients in developing and maintaining social relationships. 
    3. Supervise and assist with recreational activities and community outings. 
  3. Household Management: 
    1. Assist with light housekeeping tasks, such as laundry, meal preparation, and cleaning. 
    2. Ensure a safe and clean living environment for clients. 
  4. Documentation: 
    1. Maintain accurate and timely documentation of services provided. 
    2. Complete daily logs, incident reports, and other required documentation. 
  5. Compliance and Training: 
    1. Follow company policies, procedures, and protocols. 
    2. Participate in ongoing training and professional development. 
Qualifications
  • High School Diploma or GED 
  • One year prior professional experience preferred
  • Driver’s license preferred 
  • CPR/First Aid certification preferred
