CONGREGATE AND SUPPORTIVE HOUSING COORDINATOR


LifePath is a mission-driven, collaborative, and community-focused human services agency seeking a full-time (35 hr/wk) Housing Coordinator.


The Housing Coordinator position is in-person, on-site at the various Congregate and Supportive Housing sites in Franklin County, managing the service and care provisions for the residents. Also responsible for recruitment and assessment of housing applicants, community outreach, service planning and coordination, provision of direct individual and group services, and development of the congregate "community." The Housing Coordinator works along side the caring and skilled members of LifePath's multidisciplinary teams.

This is a full time position (M-F, 35 hrs per week); pay is $21.60 per hour, generous benefits.

QUALIFICATIONS:

  • Bachelor’s Degree in a Human Service related field.
  • Experience working with elders, individuals with disabilities or caregivers preferred.
  • Ideally possess knowledge of housing issues, service delivery systems for elders and persons with disabilities, and understanding of the aging process and disability issues, ability to establish and maintain trust and working relationships with consumers, families, staff, housing and other community agencies.
  • A self-starter with a positive and enthusiastic attitude as well as excellent oral and written communication skills.
  • Must be able to work independently as well as part of an interdisciplinary team and have the ability to problem solve and provide resource and referral options to residents.
  • Computer proficiency required.
  • Must have valid driver's license and insured automobile in good working condition; ability to visit with consumers in their own home, availability to work between the hours of 9:00 a.m. to 5:00 p.m., availability to work in the event of an emergency during off hours.

ESSENTIAL FUNCTIONS - DUTIES & RESPONSIBILITIES:

1. Recruit applicants for congregate housing through community education and outreach.

2. Facilitate Multi-disciplinary Assessment Team (MAT) as required

a. Tenant selection procedures;

b. Tenant application form;

c. Tenant termination process.

3. Develop and implement a supportive service plan for each resident including:

a. Identifying the supportive service needs and resources of each tenant;

b. Linking each tenant with appropriate community resources;

c. Reassessing the supportive service needs of tenants and revise the supportive service plan as needed.

4. Coordinate the delivery of supportive services to residents of congregate housing.

5. Facilitate open and ongoing communication, problem-solving and conflict resolution among

residents.

6. Integrate the residents into the community by:

a. Alerting residents to activities and events in the community;

b. Encouraging the use of the community space (for example, meal site, senior

center) in the congregate housing facility by others;

c. Providing information to the public about congregate and supportive housing.


Physical demands:

Visual, speaking, auditory and mobile capacity necessary:

Capacity to see computer screens, read written material, and drive a car.

Capacity to hear and speak on the telephone.

Capacity to communicate verbally with consumers, caregivers, supervisors and managers.

Capacity for fine manipulation in the frequent use of office equipment such as computers, copy machines, fax machines, telephones, calculators, etc.

Capacity to drive in all weather to visit consumers in their homes

Capacity to navigate uneven terrain in all weather to visit consumers in their homes

Capacity to climb stairs

Ability to sit and/or stand for extended periods of time.

Occasional reaching and grabbing objects with both hands, twisting of hand and wrist, and pushing and pulling of objects.

Occasional bending, squatting, and twisting to perform work functions.

Occasional capacity to lift up to 25 pounds.

Mental demands:

Ability to understand and/or interpret complicated program instructions and laws.

Capacity to maintain confidentiality of all documentation and client communications according to HIPAA standards.

Capacity to learn complicated computer software

Capacity to deal rationally and calmly with varying personalities

Capacity to work well in a fast-paced, rapidly changing environment.

Environmental/working conditions

Work is split between indoor office work and traveling outdoors to meetings, training, and consumers’ homes.

Must be able to tolerate odors and pollutants including but not limited to smoke, air fresheners, pet dander, personal odors, etc. from a variety of in-home conditions.

Must be able to tolerate by-products of office machine operation.

Must be able to tolerate heat and cold of seasonal changes and indoor temperatures.
