Position Overview:
Clinical & Support Options is seeking a dedicated Supervisor for our Low Threshold Housing (LTH) initiative.


This role will be responsible for overseeing administrative and programmatic functions for the Friends of the Homeless (FOH) Low Threshold Housing program, ensuring effective collaboration of care within the organization and with property stakeholders.

The Supervisor will address the operational requirements of housing units across various sites, working closely with the team and acting as a primary advocate and liaison for residents.


Key Responsibilities:
*As an LTH Supervisor, you will:
  • Provide leadership and guidance to case management personnel, ensuring continuous program oversight, including on-call support for urgent matters.
  • Directly oversee Housing Case Managers, facilitating training, maintaining job descriptions, managing caseload distribution, and offering constructive feedback.
  • Conduct regular one-on-one meetings with Housing Case Managers to provide supervision and assess caseloads.
  • Organize and lead regular staff meetings with team members; offer backup support as necessary.
  • Carry out probationary and annual performance evaluations for staff.
  • Ensure Housing Case Managers maintain updated records on program participants' move-in dates, relocations, or terminations.
  • Act as the primary contact for landlords, property managers, and referral sources associated with the FOH Low Threshold Program.
  • Assist in coordinating move-ins, including scheduling, key distribution, and furniture delivery.
  • Collaborate with the Director to manage all program operations, including case management and housing placement, to meet client needs effectively.
  • Maintain and update client records in compliance with agency standards and third-party requirements.
  • Ensure comprehensive documentation of tenant and supervisee information within the agency's electronic health record system.
Qualifications:
*Your background should include:
  • A Bachelor's degree (preferred).
  • Prior supervisory experience.
  • Experience working with homeless or at-risk populations (preferred).
  • Proficiency in computer applications.
  • A valid driver's license and reliable transportation.
