Duties:

Direct the coordination of patient care, under the supervision of the Clinical Manager.

Responsibilities:

1. Coordinate patient care among disciplines.

2. Confer with Clinical Manager on problems and complaints.

3. Liaison between field staff/clinicians and physician offices as necessary.

4. Perform all case management and coordinating duties as assigned

5. Review documentation to ensure accuracy and alignment with the plan of care and CMS CoPs.

6. Assist with patient intake, insurance verification, and scheduling as required.

7. Assist with scheduling, filling in where required, within the RN scope of practice.

8. Communicate with patients/families regarding care and any issues that arise.

9. Assist with supervision of staff including onboarding and orientation, performance reviews, on-site supervisory visits, and skills competencies.

10. Assist with the implementation of staff development programs and duties such as providing in-services and education required to staff.

11. Provide excellent customer service to our patients and employees.

12. Assist with management of physician orders and obtaining signatures.

13. Assist with quality assurance committees and QAPI programs along with annual overview of agency.

14. Assist with disciplinary action and remedial education as required.

15. Assist with on-call duties.

16. Performance of other duties, as required.

Qualifications:

1. Must be licensed in the state of Michigan as a Registered Nurse.

2. Must have OASIS experience.

3. 1-2 years of Home care and management experience and knowledge of federal and state home care regulations.

4. CPR certification through AHA or Red Cross.

5. Must pass a criminal background check.

6. Proof of completed Covid-19 vaccination series, negative 2-step TB test.

7. Valid driver’s license and proof of auto insurance.

8. Willing to drive up to 75 miles during supervisory/field tasks.

9. The job requires the ability to utilize a PC, calculator, multi-line telephone, and other related office equipment as well as occasional medical equipment while in the field.
