Medicare Skilled Home Care Specialist

University Home Care is seeking a skilled and knowledgeable Medicare Skilled Home Care Intake Specialist to coordinate and manage the intake process for Medicare skilled home care services.
Responsibilities:- Facilitate the intake process by working closely with patients, families, and healthcare providers to ensure timely and accurate enrollment in our program.
- Gather and review necessary documentation to determine eligibility and medical necessity for skilled home care services.
- Communicate with insurance companies and other third-party payors to obtain authorization for services.
- Coordinate with our clinical team to develop and implement customized care plans for each patient.
- Educate patients and families about Medicare coverage and our services.
- Ensure all necessary paperwork and documentation is completed accurately and in a timely manner.
- Maintain accurate and organized electronic records of all intake activities.
- Collaborate with other members of the team to streamline processes and improve overall efficiency.
- Adhere to all Medicare and Medicaid regulations and guidelines.
- Participate in ongoing training and development opportunities to stay up-to-date on changes in policies and procedures.
- High school diploma or equivalent, some college coursework in healthcare or related field preferred.
- At least 1 year of experience in healthcare intake or related role, specifically with Medicare and/or Medicaid.
- Excellent communication skills, both verbal and written.
- Strong organizational skills and attention to detail.
- Ability to work independently and in a team environment.
- Proficient in Microsoft Office and electronic medical record systems.
- Knowledge of Medicare and Medicaid regulations and guidelines.
- Compassionate and empathetic attitude towards patients and families.
