Position Summary
The Outreach Specialist supports program growth by making initial contact with potential patients and partners, coordinating sobering placement, and enrolling patients into a designated health plan. The Outreach Specialist coordinates crisis care for the most vulnerable populations in the greater Sacramento Region. The goal of this service is to provide a safe, community response to a substance use and behavior health crisis. The Outreach Specialist is responsible for keeping detailed records of their efforts and communicating regularly with Supervisors and Program Managers.
Required Qualifications
- High School diploma or equivalent. Bachelor’s degree preferred.
- 2 years of case management, crisis care, or outreach experience, and/or lived experience utilizing Medicaid or other social service systems.
- Must be able to work collaboratively with others in a team environment, respecting and valuing the
- perspectives and experiences of others.
- Familiar with Harm Reduction, Motivational Interviewing, Client Centered theory and Trauma Informed Care.
- Able to maintain and understand appropriate roles, and boundaries among patients, peers, and WellSpace partners/stakeholders.
- Able to function effectively in a fast paced environment.
- Ability to write progress notes and maintain detailed, statistical records.
- Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, people living with HIV (AIDS), and/or physiologically impaired.
- Must demonstrate a high level of verbal, writing, and listening skills.
- Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
- Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment.
- Computer literacy including use of email, Microsoft Word, Excel, and EHR.
- Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
- Must have valid driver’s license and insurable driving record. Must use your vehicle for business-related travel, including community meetings. Eligible for mileage reimbursement.
Essential Responsibilities
- Reach out in-person, by phone, and email to explain Crisis Receiving for Behavioral Health Services and educate the community on the referral process.
- Manage daily intakes.
- Confirm participation or enroll patients into the appropriate Managed Care Plan.
- Coordinate referral process to Cal Aim Services according to established procedures.
- Keep detailed records of efforts, regardless of outcome, including spreadsheets and reports.
- Document all interactions with and on behalf of the patient in the appropriate patient care portals including, the patient’s Electronic Health Record (EHR).
- Participate in care coordination meetings with various partners as deemed appropriate.
- Work as part of a multi-disciplinary team including LVNs, Clinicians, and Behavioral Health Specialists.
- Attend regular meetings with management and the team.
- Practice mandated reporting/incident reports based on agency and state guidelines.
- This position will require travel between WellSpace Health locations as well as to other community locations as needed.
- Participate and complete all job-related training, certification, and renewals.
- Perform other duties as assigned.

Physical Demands and Work Environment
The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the patient’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
