Population Health Manager

Reporting to the Director of Managed Care, the Population Health Manager leads and facilitates clinical improvement projects, assists with improvement measurement and analysis, and assists with reporting of clinical outcomes and improvements. Integrates improvement support activity with related regulatory and accreditation standards to promote compliance.
DUTIES AND RESPONSIBILITIES:
- Lead population health program implementation and deliverables, as undertaken by the clinic.
- Interact with external stakeholders and represent AHSFHC in meetings pertaining to population health initiatives.
- Interact with health plans for managed care initiatives.
- Utilizes quality improvement techniques to improve customer care and clinical outcomes.
- Generates clinical performance and care gap reports and incorporate measures to improve patient outcomes.
- Partake in health education and other direct patient facing initiatives offered by AHSFHC.
- Incorporates regulatory accreditation requirements into improvement projects.
- Abstracts data from medical records for key clinical improvement projects.
- Research trends in healthcare quality improvement and lead efforts to plan and develop improvement initiatives.
- Assists with identifying benchmarks and comparisons for clinical outcomes measures.
- Advises and assists leadership on matters affecting quality and improvement.
- Accepts other duties as assigned to promote the accomplishment of organizational goals.
- Coordinate and implement patient satisfaction survey at all clinic sites.
- Work in coordination with AHSFHC CQI staff in maintaining Patient Centered Medical Home (PCMH) certification
- Plan and prepare the policies and procedures to meet the standards of care for NCQA’s Patient Center Medical Home (PCMH) accreditation.
- Achieve and maintain regulatory and voluntary standards and accreditation relating to quality systems required for support of AHSFHC business practices.
- Other duties as assigned by supervisor.
PREFERRED QUALIFICATIONS:
1. Familiarity with managed care and the healthcare space.
2. Ability to collect, analyze, and present statistical data;
3. Communicate effectively orally and in writing with health care professionals and the general public.
4. Prior experience in a healthcare setting, and demonstrated abilities for working in a multi-cultural setting.
5. Proficiency in Microsoft Office: Word, Excel, PowerPoint.
MINIMUM QUALIFICATIONS:
1. B.S./B.A. in social work, social/health sciences.
2. Illinois RN License, or MPH is preferred.
3. Valid drivers’ license and insured automobile.
4. Bilingual in one Asian language or Spanish is preferred
