Job Title: APRN/PA I Population Health

Cleveland Clinic Florida is seeking a compassionate and hard-working Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) to join our team in Population Health. The ideal candidate will have ambulatory experience, preferably in family or internal medicine, and be comfortable handling a wide range of responsibilities throughout the day.

Job Summary:

The APRN/PA I Population Health will work in collaboration with a physician to provide primary and specialty care to adult patients in a variety of healthcare settings. The successful candidate will have excellent analytical and decision-making skills, strong written and verbal communication skills, and the ability to work as a member of a multi-disciplinary team.

Responsibilities:
  • Conduct thorough medical histories, perform complete physical examinations, and initiate appropriate lab and radiology tests to establish a record of the patient's current health status and develop a working diagnosis and treatment plan.
  • Work collaboratively with a physician to provide high-quality patient care.
  • Perform routine procedures such as injections, immunizations, and wound care, as well as assist in the management of more complex illnesses and injuries.
  • Prescribe and monitor medications as allowed by prescriptive authority.
  • Participate in medical team rounds and collaborate with nursing, medical, and other healthcare team members to ensure quality patient care.
  • Refer patients to specialists as appropriate for consultation or for specialized health resources and treatment.
  • Ensure continuity of care by serving as a liaison between patients and other members of the multi-disciplinary care team.
Requirements:
  • APRN: MSN required upon hire or within 5 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role. Graduate of an accredited APRN graduate program.
  • PA: Graduate of an ARC-approved Physician Assistant Program. Master's degree in Physician Assistant Studies and/or applicable state license through the State Medical Board for certificate to practice.
  • Current certification as an APRN or PA through a nationally accredited organization recognized by the state Board of Nursing or Medicine.
  • Basic Life Support (BLS) through the American Heart Association (AHA) required.
  • Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) through the American Heart Association (AHA) may be required based upon work setting.
Competencies:
  • Excellent analytical and decision-making skills.
  • Strong written and verbal communication skills.
  • Ability to work as a member of a multi-disciplinary team.
  • Demonstrated competency and the ability to perform advanced assessment and diagnosis.
Work Experience:
  • APRN: Two years clinical nursing experience preferred. Will consider new graduates.
  • PA: Two years relative clinical experience preferred. Will consider new graduates.
Physical Requirements:
  • Typical physical demands include the ability to walk and stand for long periods of time.
  • Manual and finger dexterity and eye/hand coordination to perform physical examinations.
  • Requires corrected vision and hearing to a normal range.
  • Occasionally lifts and carries items weighing up to 50 pounds.
