The Down Syndrome Association of Wisconsin is a statewide non-profit organization dedicated to advancing the lives of individuals with Down syndrome and related disabilities and their families through awareness, education, information, programs, services, and the exchange of ideas and experiences.

 

DSAW provides programs and services throughout the lifespan (Parent’s First Call for new and expectant parents, children’s services, adult services, vocational services, and aging services), as well as educational opportunities and events.


The Long Term Support Specialist will oversee a caseload of participants utilizing DSAW for long-term job coaching services, including providing case management, working with job coaches on the caseload, and providing job coaching themselves. In addition, the Long Term Support Specialist will play an integral role in growing DSAW’s long-term job coaching services through attendance at events and other business development activities. 

 

The Long Term Support Specialist will have an important role in all aspects of case management including communicating with families and funding sources, staying within budgets and providing quarterly reports to both families and funding sources. The Long Term Support Specialist will also confirm monthly coaching hours for each participant to our billing department. On the participant side, this includes working with the individual and their family to create goals in the workplace, scheduling services, providing services, and ensuring ongoing quality of services with onsite check in’s. The Long Term Support Specialist will also assist with any other job coaching needs by the Employment Specialists. 


The ideal candidate should have experience working with individuals with disabilities. This person should have excellent organization and prioritization skills in order to assist with case management. The Long Term Support Specialist must be a skilled communicator, empathetic listener, and excellent problem-solver, with the ability to help clients and their families put plans in place to achieve their desired goals. Overall, the candidate must be  passionate about helping individuals with diverse abilities achieve their employment goals. 

 

In order to complete this job well, the Long Term Support Specialist must be very comfortable with technology, and have the ability to quickly learn new systems, such as a new Case Management System. The Long Term Support Specialist should have a thorough understanding of MS Office/G Suite products. Experience with a client management system and a time tracking/scheduling system is a plus.

Essential Functions and Responsibilities 

Participant Case Management

  • Participate in the initial transition meeting from DVR to Long Term Support for new participants
  • Oversee participant goal setting
  • Maintain Case Management System with updated goals, reports, and records
  • Work with the Job Coach Scheduler to ensure a good fit between the participant and job coach
  • Provide periodic check in with the participant and Job Coach in their workplace
  • Provide quarterly reports for family and funding sources
  • Provide responsible oversight of participants available coaching hours
  • Prepare and confirm monthly coaching report for billing

Staff Support

  • Provide feedback and suggest strategies and modifications for Job Coaches
  • Oversee quality of services by Job Coaches with onsite, periodic check-ins
  • Manage staff scheduling in partnership with Job Coach Scheduler
  • Schedule Meet and Greets for new participants to introduce Job Coach

 Regional Growth

  • Work with the Employment Services Manager on big-picture strategy for regional growth
  • Help grow services through activities such as outreach to families and funding sources, attending relevant DSAW and community events, etc
  • Work with the Employment Services Manager, Southeast Wisconsin and the Director of Programs to meet quarterly and annual goals for the region 

Direct Services Provision

  • Consistently model appropriate soft and hard skills in the work environment
  • Provide on site job coaching to consumers
  • Support client by practicing, modeling and guiding them through real life experiences
  • Maintain and model positive professional working relationships with clients, guardians, employers, and funding sources
  • Advocate for clients as needed and teach advocacy skills
  • Attend required meetings and in-service training sessions
  • Monitor client’s goals and complete progress notes 
  • Other duties as assigned

Qualifications

Education/Experience

  • Highschool diploma or equivalent 
  • Bachelor’s degree in a related field preferred
  • 2+ years working with families and clients with diverse abilities
  • Experience working with DVR, IRIS and/or Family Care
  • Experience with all aspects of job coaching (prior job coaching experience preferred)
  • Applicant must have a valid driver’s license and insured vehicle
  • Applicants must pass a caregiver background check that meets State of Wisconsin requirements

  • Demonstrates warmth, insight, interest, and respect for persons with disabilities
  • Willingness to accept flexibility with their schedule

  • Ability to multi task and adhere to timelines set forth for quality management


Compensation

This is an hourly position budgeted at $19-$21 / hour.
