Position: Employment Support Specialist for Long-Term Services

Location: Southeastern Wisconsin (25% remote, 75% providing services and case management throughout the region)

Reports To: Employment Services Manager

Status: Full-time with variable hours

Compensation: $19-$21 / hour, based on experience

Organization Overview: The Down Syndrome Association of Wisconsin is a dedicated non-profit organization focused on enhancing the lives of individuals with Down syndrome and related disabilities through awareness, education, and comprehensive support services.

We offer a variety of programs and services throughout the lifespan, including vocational services, educational opportunities, and community events.

Role Summary: The Employment Support Specialist will manage a caseload of participants utilizing our long-term job coaching services. This role includes providing case management, collaborating with job coaches, and delivering direct job coaching services. The Specialist will also contribute to the expansion of our job coaching services through participation in outreach and business development initiatives.

Key Responsibilities:

  • Oversee case management for participants, ensuring effective communication with families and funding sources.
  • Assist in goal setting for participants in their employment journey.
  • Maintain accurate records and reports within the Case Management System.
  • Conduct regular check-ins with participants and job coaches to monitor progress.
  • Prepare and submit monthly coaching reports for billing purposes.

Qualifications:

  • High school diploma or equivalent; Bachelor's degree in a related field preferred.
  • Minimum of 2 years of experience working with individuals with diverse abilities.
  • Familiarity with job coaching and case management processes.
  • Valid driver's license and insured vehicle required.

  • Strong organizational and prioritization skills.
  • Excellent communication and interpersonal abilities.
  • Proficiency in technology and software applications, including MS Office and client management systems.
