Job Description
Patient Centered Services, LLC is looking for Clinical Pharmacists to fill front-line medication verification and dispensing roles for our inpatient pharmacy operation in a pediatric hospital. The ideal candidate will be responsible for ensuring the safe, effective, and timely distribution of medications to patients, working closely with the healthcare team to provide comprehensive pharmaceutical care.
Responsibilities
- Verify and dispense medications accurately and efficiently.
- Review and interpret physician orders to ensure appropriate therapy.
- Provide drug information and education to healthcare professionals, patients, and families.
- Monitor patient medication therapies for effectiveness, side effects, and potential interactions.
- Participate in multidisciplinary rounds and collaborate with healthcare teams to optimize patient outcomes.
- Maintain accurate records of medication dispensing and patient interactions.
- Ensure compliance with all regulatory requirements and hospital policies.
- Utilize EHR platform Cerner and Production Platform DoseEdge to manage and document pharmacy operations.
- Assist in the training and supervision of pharmacy technicians and interns.
- Licenses and Education:
- DC Pharmacist License required for Clinical Pharmacist.
- PharmD requirement for the Clinical Pharmacist role.
- Experience:
- Inpatient hospital experience preferred.
- Experience with EHR platform Cerner and Production Platform DoseEdge favorable.
- Pediatric experience a plus but not a requirement.
- Strong knowledge of pharmacology, therapeutics, and clinical pharmacy practices.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced, team-oriented environment.
- Strong attention to detail and problem-solving skills.
- Ability to handle multiple tasks and prioritize workload.
- Commitment to continuous learning and professional development.
- Shifts: 8 hours with rotating cadence that can include the following shift times:
- AM (6:00 AM - 2:30 PM)
- PM (2:00 PM - 10:30 PM)
- Availability: Weekday and Weekend shifts required.
