The Reentry Navigator, Partners In Hope, guides individuals transitioning from incarceration to successful community reintegration. In a faith-based context, this role fosters client growth, facilitates connections with services, and collaborates with partners for adequate reentry support.

Key Responsibilities:

 

· Client Support & Advocacy: Provide client-centered care, offering informed guidance and assistance in creating tailored service plans.

· Case Management: Maintain accurate records, identify needs, and manage a caseload of active participants.

· Post-Release Care: Monitor progress, offer one-on-one support, and facilitate group sessions during key reentry phases.

· Reentry & Career Development: Arrange essentials, manage financial matters, assist with benefits setup, coordinate with employment navigator.

· Resource Coordination: Address housing, transportation, employment, and counseling needs, offering emergency financial aid when possible.

· Partnerships: Liaise with reentry service providers and community organizations to connect clients with vital resources.

 

Qualifications:

· Full-time availability with flexibility for evenings and weekends.

· Proficiency in Microsoft Office and case management software.

· Strong organization and record-keeping skills.

· Understanding of case management processes and substance use disorders.

· Knowledge of workforce entry barriers and problem-solving for reentry individuals.

· Effective prioritization under compassion and patience.

· Excellent communication and presentation abilities.

· An associate degree is preferred; reentry experience can be a substitute.

· Previous case management experience preferred.

· Valid Driver's License with reliable transportation.

 

Working Conditions:

· Extended computer use.

· Adapt to non-typical office environments, including correctional institutions.

· Flexibility for occasional nights and weekends.

· Ability to handle stress and interpret verbal communication.
