The Early Childhood Development (ECD) Patient/Family Education Specialist (PFES) is responsible for working with families to help them realize their self-sufficiency goals. He/she engages families in their child/ren(s) learning activities and serves as the link between families and community resources. The PFES will assist in the identification, screening, care, and referral of children and adolescents with developmental disabilities/delays and co-occurring psychiatric or behavioral disorders. In addition, the PFES will support the development of systems and infrastructure to increase support services within the PanCare pediatric and behavioral health departments and other local community systems. The PFES will provide services and support to meet the needs of their patients and their family/care givers.

Duties and Responsibilities:

  • Conducts outreach and community education to identify children and their family members who are in need of ECD screening.
  • Assist parents/guardians with identifying their individual and family strengths and needs.
  • Assist families with developing family service plans.
  • Link families to needed community resources.
  • Assess and evaluate family success in reaching their goals (outcomes).
  • Coordinate services when families are connected with more than one service.
  • Follow up with families to assess goal attainment or to adjust/adapt goals, as necessitated.
  • Maintain family files ensuring all required documents are obtained and all signatures documented.
  • Enter child and family data into the electronic health record.
  • Ensures accuracy of data and ensures data is entered timely.
  • Analyzes and reports on data, as required.
  • Establishes collaborative relationships with community organizations.
  • Verifies eligibility for the program by collecting, reviewing, and maintaining required documents.
  • Monitors enrollment and attendance at appointments and follows up on missed appointments.
  • Provide support to staff and work collaboratively for the well-being of the children and families served.
  • Conduct parent education and other training for parents/guardians.
  • Participate in daily team huddles, staff meetings and trainings.
  • Conducts home visits with families for the purpose of establishing rapport, obtaining paperwork, setting and following through with goals, assuring that children’s records are obtained and providing crisis assistance, as needed.
  • Performs other duties as required.
  • Participates in all safety programs, which may include assignment to an emergency response team.
  • Other responsibilities as assigned to create a smoothly run, efficient clinic.

Qualifications:

  • Bachelor’s degree in Social Work, Counseling, or related field, or Bachelor’s degree with two (2) years’ experience working with high risk families.
  • Knowledge and skills in community-based behavioral health care and case management a plus.
  • Excellent interpersonal and communication skills.
  • Current driver's license with clean record
