As a key member of our multidisciplinary team, you will play a vital role in providing expert guidance and education to patients with diabetes in both in-person and virtual outpatient settings.
**Key Responsibilities:**
- Conduct in-person and virtual consultations with diabetic patients to assess their educational needs and develop personalized care plans.
- Provide education on diabetes management, including medication administration, blood glucose monitoring, nutrition, exercise, and lifestyle modifications.
- Utilize virtual healthcare platforms to deliver education and support to patients remotely, ensuring seamless continuity of care.
- Collaborate with physicians, nurses, dietitians, and other healthcare professionals to coordinate comprehensive care for diabetic patients.
- Monitor patient progress, adjust care plans as needed, and provide ongoing support to ensure successful diabetes management.
- Maintain accurate and up-to-date patient records, documentation, and reports.
- Stay current with the latest developments in diabetes care and education to provide the highest quality of service.
- Participate in community outreach programs to raise awareness about diabetes prevention and management.
**Requirements:**
- Bachelor's degree in Nursing, Dietetics, Public Health, or a related field; Master's degree preferred.
- Certified Diabetes Educator (CDE) or Diabetes Care and Education Specialist (DCES) certification required.
- Minimum of 2 years of experience in diabetes education or related clinical practice.
