  • Utilize virtual healthcare platforms to deliver education and support to patients remotely, ensuring seamless continuity of care.
  • Collaborate with physicians, nurses, dietitians, and other healthcare professionals to coordinate comprehensive care for diabetic patients.
  • Monitor patient progress, adjust care plans as needed, and provide ongoing support to ensure successful diabetes management.
  • Maintain accurate and up-to-date patient records, documentation, and reports.

**Requirements:**

  • Bachelor's degree in Nursing, Dietetics, Public Health, or a related field; Master's degree preferred.

  • Minimum of 2 years of experience in diabetes education or related clinical practice.
