The Parent-Child Assistance Program (PCAP) is a paraprofessional intervention for high-risk women and their families. This position will be responsible for providing direct outreach, home visitation case management, and advocacy services to high-risk mothers who have used alcohol and/or drugs heavily during pregnancy, and to their children, partners, and extended families when needed.
*Job located in Bremerton, Washington. This position is NOT remote-work eligible. This position is NOT eligible for a moving reimbursement to relocate to area. Applicants out of area MUST be willing to move at their expense if beyond commuting range*
RESPONSIBILITIES
A. Perform case management activities according to PCAP policies and procedures, in a highly confidential and professional manner.
B. Manage caseload of 15-20 clients and their families.
C. Conduct home visits as clinically necessary (approximately twice per month).
D. Assist clients in setting goals based on their individual needs assessments, and in identifying steps to achieve their goals; monitor progress.
E. Identify high-quality community providers and agencies whose services are relevant to client needs.
F. Provide outreach services to engage and connect clients with the community agencies that will help meet their needs and goals.
G. Administer clinical assessment instruments dealing with highly sensitive and confidential information, according to PCAP policies and procedures.
H. Collect and enter data online.
I. Coordinate scheduling and attend case staffings and appointments with community providers with whom client is involved. Maintain regular contact with client's network of service providers.
J. Provide housing support/ case management services and documentation as required for housing subcontracts and working agreements.
K. Attend and participate in weekly individual supervision with site Clinical Director to discuss client needs, goals and issues.
L. Participate in weekly staff meetings with team to evaluate client and program progress.
M. Provide outreach services to locate missing clients and engage them with the program.
N. Participate in required training.
O. Obtain and maintain current CPR, Infant CPR, and First Aid Certification.
P. Contribute to report writing, trainings, and presentations as needed.
Q. Other duties as assigned.
REQUIREMENTS
A. 4 Year BA or BS degree and 4 years of community-based experience working with high risk populations.
B. Experience on issues/projects working with prenatal substance abuse or equivalent combination of education and work experience.
- Strong communication skills.
- Demonstrated ability to effectively engage with high-risk mothers and their families.
- Applicants shall conduct themselves ethically and professionally at all times, adhering to the agency Code of Ethics.
C. Ability to conduct interviews covering extremely sensitive and confidential issues.
Ability to work both as a team member and independently. Possess strong communication skills, both written and verbal.
- Ability to maintain a positive relationship with clients and to be of the conviction that Substance use is a treatable illness and constitutes a public health problem.
- Knowledge of substance use and its effects on individuals, families, and communities is required. Knowledge of relapse prevention and crisis management skills.
- Knowledge of local resources for children and families preferred.
- Possess computer skills, minimum of Microsoft Word, Excel and able to organize schedules and files are essential.
- Knowledge of community/recovery resources.
- Be able to pass required background check.
- Must have a valid Washington State driver’s license, a clean driving record and be able to travel on a daily basis (company vehicle provided).
- Ability to meet physical and training requirements of the position.
- Applicants shall understand and adhere to state and federal confidentiality regulations.
- Must sign an oath of permanent confidentiality covering all patient-related information.
- Within six months of hire, obtain CPR, First Aid, Blood Borne Pathogen training certificates.
- Need proof of clean driving record and a valid WDL
