About the PACT Team

Kitsap Mental Health Services' Program for Assertive Community Treatment (PACT) Team offers intensive, team-based behavioral health services with highly individualized support within the community setting. PACT provides high-intensity services designed for clients with severe, chronic mental illnesses that interfere with daily life.

Primary Responsibilities

Provide treatment coordination services for an assigned group of clients, participating in scheduled treatment team meetings.
Develop, implement, evaluate, and revise overall treatment goals and plans in collaboration with clients and the Individual Treatment Team (ITT), using recovery-oriented language.
Utilize cognitive behavioral approaches, motivational interviewing, trauma-informed care, and stage-wise treatment strategies to motivate clients for change.
Provide comprehensive case management services addressing housing, health, hygiene, communication, finances, food sufficiency, legal requirements, social skills, recreation, ADLs, and vocational goals.
Educate and support clients' families, advocate for clients' rights, and consult with community agencies to maintain treatment coordination.
Participate in the PACT Comprehensive Assessment process for assigned clients.
Document client progress according to established policies and procedures.
Participate in daily staff organizational meetings and scheduled treatment planning review meetings.
Provide psychiatric rehabilitation services, assisting clients in building skills for independent living, including shopping, cooking, cleaning, personal hygiene, budgeting, socialization, and accessing transportation.

Minimum Qualifications

EDUCATION: Bachelor's Degree in Psychology, social services, or behavioral health field.
EXPERIENCE: Entry level (no prior related work experience).
This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation.
LICENSURE: Agency Affiliated Counselor Registration. (Can be obtained after hire)

EXPERIENCE: Experienced (minimum 2 years of job-related experience)
Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment
