Job Description: We are seeking a highly skilled and compassionate Per Diem Registered Nurse (RN) to join our team. The primary responsibility of this role is to conduct care plan assessments for our clients on an on-call basis. The ideal candidate will have a strong background in home health care, excellent assessment skills, and the ability to develop individualized care plans that meet the needs of our diverse client population.
Responsibilities:
- Conduct comprehensive care plan assessments for new and existing clients.
- Develop, implement, and update individualized care plans based on client needs.
- Collaborate with clients, families, and other healthcare professionals to ensure optimal care.
- Monitor and document client progress and any changes in health status.
- Provide education and support to clients and their families regarding care plans and health maintenance.
- Ensure compliance with all state and federal regulations and company policies.
- Maintain accurate and up-to-date medical records.
Qualifications:
- Valid Registered Nurse (RN) license in the state of New Jersey.
- Minimum of 2 years of experience in home health care or a related field.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and willingness to travel within Bergen County.
- Flexibility to work on-call and respond to assessment requests promptly.
