We are seeking a compassionate and skilled Registered Nurse (RN) to join our client’s Certified Home Health Agency (CHHA) team. The RN will provide high-quality, patient-centered care to a diverse geriatric and long-term care population within their homes. This role involves conducting comprehensive assessments, developing individualized care plans, and delivering direct nursing care, ensuring our clients achieve their highest possible level of health and comfort.

Key Responsibilities:

  • Patient Assessment: Conduct thorough initial and ongoing assessments of the physical, psychosocial, and environmental needs of geriatric and long-term care patients.
  • Care Planning: Develop, implement, and regularly update individualized care plans tailored to the unique needs of each patient, in collaboration with the healthcare team.
  • Nursing Care: Provide skilled nursing care, including wound care, medication administration, pain management, and monitoring of chronic conditions, ensuring adherence to care plans and physician orders.
  • Patient Education: Educate patients and their families about the disease process, medications, treatment options, and home care techniques to promote self-management and well-being.
  • Collaboration: Work closely with physicians, social workers, physical therapists, and other healthcare professionals to coordinate comprehensive care and optimize patient outcomes.
  • Documentation: Maintain accurate and timely documentation of patient care, assessments, interventions, and progress notes in compliance with regulatory requirements and agency policies.
  • Compliance: Ensure all care is provided in accordance with state and federal regulations, as well as CHHA policies and procedures.
  • Emergency Response: Recognize and respond to changes in patients' conditions, initiate appropriate interventions, and communicate effectively with the healthcare team and emergency services as needed.
  • Quality Improvement: Participate in continuous quality improvement initiatives and contribute to the enhancement of care delivery and patient satisfaction.

Requirements:

  • Current and unrestricted Registered Nurse (RN) license in the state of NY.
  • Minimum of 1-2 years of nursing experience, preferably in home health, geriatric care, or long-term care settings.
  • Graduate of an accredited nursing program; BSN preferred.
  • Strong clinical assessment skills, excellent communication and interpersonal abilities, proficiency in electronic health records (EHR) systems.
  • Compassionate, patient-centered, and able to work independently while managing multiple tasks.

Please note- The annual base salary range is $75k to $95k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
