- Provide counseling and emotional support to patients and families.
- Facilitate discussions about end-of-life issues, grief, and loss.
- Assist in addressing psychological and social issues related to the patient's condition.
Resource Coordination:
- Identify and connect patients and families with community resources, such as financial assistance, support groups, and legal aid.
- Help patients and families navigate healthcare systems and access appropriate services.
Crisis Intervention:
- Respond to crises, such as acute emotional distress or family conflicts.
- Develop strategies to manage and resolve these crises effectively.
Education and Advocacy:
- Educate patients, families, and caregivers about hospice philosophy and services.
- Advocate for patient rights and wishes, ensuring they are respected and followed.
Interdisciplinary Collaboration:
- Collaborate with the hospice team, including doctors, nurses, chaplains, and other professionals, to provide holistic care.
- Participate in team meetings, case conferences, and care plan evaluations.
Documentation:
- Maintain accurate and up-to-date records of patient assessments, care plans, interventions, and progress notes.
- Ensure compliance with hospice policies, procedures, and regulatory requirements.
Bereavement Support:
- Provide follow-up bereavement counseling and support to families after the patient's death.
- Organize and lead bereavement support groups or memorial services.
Qualifications:
Education:
- Master’s degree in Social Work (MSW) from an accredited institution.
- Licensed Clinical Social Worker (LCSW) preferred.
Experience:
- Previous experience in a hospice or palliative care setting is highly desirable.
- Experience with grief counseling, crisis intervention, and family dynamics.
Skills:
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient and family care.
- Knowledge of community resources and healthcare systems.
