Company Overview:
LivWell Community Health Services is a non-profit organization providing comprehensive
health and support services, education, and advocacy, related to HIV/AIDS, sexually transmitted
diseases, and viral hepatitis. Quality health care is provided to persons living with HIV/AIDS or
sexually transmitted diseases, support services are provided to HIV clients and their families,
and prevention education and testing are provided to all clients, their families, and the
community at large. Within the community, we strive to prevent new infections of HIV and
STIs and reduce the stigma associated with these various diseases by promoting awareness,
acceptance, and equality.
Position Overview:
The Linkage to Care Navigator is responsible for providing direct case management for
incarcerated individuals within the service area. Case management includes administering
risk/need and other assessments, providing reentry support, responding to inmate service
needs during their incarceration, etc. This individual works closely with community providers
specific to the jail system(s). This includes but is not limited to going into jails to assist with
discharge planning, working with the client to eliminate any housing barriers upon release,
advocating on the client's behalf with the court system, and or with Probation and Parole. The
Linkage to Care Navigator is also a critical aspect of the intake team and process for individuals
seeking services for HIV care. This position identifies and manages barriers to care to obtain
sustainable medical adherence to promote optimal health outcomes. The Linkage to Care
Navigator will facilitate in completing all program-specific paperwork, assisting in obtaining
medical insurance when applicable and assisting in the elimination of barriers that are deemed
urgent per program standards.
Key Responsibilities:
• Develop professional relationships necessary to guide the client into medical care until
they are fully involved in care and enhance health outcomes
• Assists clients in understanding programming paperwork and navigating HIV care
• Establishes and maintains individual and group relationships with community-based
programs, associations, businesses, citizens and citizen groups, and individual educational
programs to enhance program services to the Jail-Active population

• Work with the Intake Specialist and Intake Nurse to schedule an appointment for the
client to complete Medical/Part B intake
• Work closely with the housing staff to alleviate any housing needs/barriers
• Maintain a small caseload of clients who are Jail-Active (incarcerated individuals)
• Collaborates with case managers to identify clients who are HIV-positive and at risk of
dropping out of care or not receiving care
• Develop individual care plans to improve the client’s quality of life including housing,
education, employment services, and basic needs
• Discharging planning and housing stability assistance
• Empower clients to live healthy lifestyles and provide the tools needed to manage their
health and understand risk-reduction methods
• Collect, sort, and submit various types of data from state and interagency resources.
• Accompany client to appointments, if necessary
• Any other duties as assigned
Required Qualifications:
• Bachelor’s degree in Social Work, Human Services, or related field.
• One year of social work experience in a health-related field.
• Willingness to become a certified HIV tester in the state of Kentucky.
• Experience in social work, human services, or a similar setting
Preferred Qualifications:
• Two years of social work experience in a health-related field preferred.
• Experience with incarcerated individuals preferred.
• Experience in HIV care, support services, outreach, health promotion, and/or similar
fields
• Ability to lift event supplies, documents, testing supplies, etc., up to 25 pounds.
