Position Title: Targeted Case Manager
Position Summary: Emerald is in search of dedicated individuals who are passionate about assisting clients in recognizing their inherent strengths to navigate obstacles. Targeted Case Management emphasizes leveraging strengths and capabilities to enhance skill sets, expand social networks, and connect clients with essential community resources. This may encompass access to housing, medical care, social services, education, and other necessary supports. Clients may present with Substance Use Disorders (SUD), Severe Mental Illness (SMI), or Severe Emotional Disabilities (SED).
This role involves managing a caseload of up to 25 clients, which includes direct client interactions, collateral communications, travel, documentation, planning, and resource development. The position comprises 50% direct service, 25% documentation, 15% knowledge and skill enhancement, and 10% administrative tasks.
Key Responsibilities:
- Accept newly assigned cases following diagnostic intake by Emerald providers.
- Conduct comprehensive assessments and periodic reassessments for all clients on the caseload.
- Collaborate with clients to develop and implement tailored treatment plans.
- Provide ongoing case management that aligns with the client's needs and goals as outlined in the treatment plan.
- Adjust treatment plans as necessary to address emerging client needs.
- Engage in planning and coordination activities to enhance service continuity and effectiveness.
- Maintain open communication with intake, medical, and counseling teams regarding client progress.
- Prepare and manage client records, including treatment plans and progress documentation.
- Demonstrate proficiency in interviewing and assessing client needs.
- Identify and connect clients with valuable community resources.
- Foster positive working relationships with internal staff and community partners.
- Ensure compliance with Kentucky Medicaid and accreditation standards in all documentation.
- Stay informed about state reporting requirements for dependency, abuse, and neglect.
- Utilize coping skills and de-escalation techniques to enhance client motivation.
- Work independently while effectively prioritizing tasks.
- Participate in weekly staff meetings.
- Provide case management services for a maximum of 25 clients simultaneously.
- Perform additional duties as assigned by the supervisor, consistent with agency policies.
Qualifications:
- Bachelor's degree in Social Work, Sociology, Psychology, or a related field.
- Completion of the targeted case management training program within six months of hire and adherence to certification requirements.
- Strong communication skills, technological proficiency, and community engagement abilities are essential.
- Flexibility in scheduling to meet client needs.
- Access to reliable personal transportation.
