JOB SUMMARY:

The CCBHC Behavioral Health Case Manager is a member of the behavioral health/family health services team(s). The teams are composed of multidisciplinary professionals including; Behavioral Health Clinicians, Behavioral Health Peers, Primary Care Providers, Psychiatric Providers, Nurses, Medical Assistants and Social Workers. The CCBHC Behavioral Health Case Manager works closely with Peers Support Specialists, primarily in the community, to support the needs of behavioral health clients and patients. The CCBHC Behavioral Health Case Manager helps clients increase their penetration into the community health system of care and access to quality behavioral care. This is done through outreach and engagement, professional relationship building, and assistance with coordination of care plans.

WORK SCHEDULE DEMANDS:

  • Full-time; 40 hours per week position
  • Due to the nature of the job, this individual must be flexible with their schedule to accommodate the client population.
  • May be required to work at any CCAP location.


CONFIDENTIALITY OF INFORMATION:

Has access to program case management records. Maintains confidentiality of all material in accordance with State and Federal law and CCAP policies and procedures.


KEY RESPONSIBILITIES:

  • To support the treatment service plan with clients;
  • Meet with clients at CCAP Behavioral/Family Health Services sites and in the community;
  • Coordinate services to clients appropriately utilizing various components of CCAP as well as referrals to other local or state agencies;
  • Act as a liaison between the family, legal system, and any resources in the community, statewide;
  • Engage individuals who may be at risk and coordinate crisis risk response with behavioral health clinicians, providers, and office staff;
  • Assist in the orientation process for new clients requiring mental health and/or co-occurring disorder services;
  • Assist clients to reduce high risk behaviors and health risk factors such as smoking, poor illness self-management, inadequate nutrition, and infrequent exercise in order to create and maintain a healthy lifestyle. (health promotion);
  • Coordinate with colleagues to help clients make behavioral changes leading to positive lifestyle improvement. (individual and family support services)
  • Introduce and link wellness and healthy lifestyle resources to assist clients in achieving behavioral health goals and objectives;
  • Educate clients on the behavioral health system and how to most effectively navigate it;
  • Attend and participate in all CCAP public relations activities as appropriate.
  • Attend training, conferences, and professional development sessions;
  • Schedule and attend weekly supervision meetings with supervisor;
  • Other duties as assigned.

Requirements:

REQUIRED QUALIFICATIONS:

  • Bachelor’s degree in social work, human services, related field, or equivalent work and lived experience;
  • Excellent oral and written communications skills;
  • Experience using an Electronic Health Record (EHR) preferred;
  • Knowledge of the health and human services infrastructure, health insurance programs and public coverage options;
  • Ability to effectively develop and nurture relationships with a diverse group of stakeholders;
  • Ability to work independently and coordinate multiple tasks;
  • Ability to enthusiastically communication with potential enrollees;
  • Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications and the Microsoft operating system.


MINIMUM QUALIFICATIONS:

  • Must be able to travel with use of a personal vehicle (Must have driver's license, must have automobile insurance);
  • Must possess the ability to read and interpret documents;
  • Must possess the ability to write routine reports and correspondence;
  • Must possess the ability to speak effectively before groups and actively engage the general public in outreach situations;
  • Must possess the ability to interact respectfully with diverse cultural and socio-economic populations.

This position requires prolonged sitting, some bending, stooping, and stretching. It requires eye-hand coordination, and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment. It also requires a normal range of hearing and eyesight to record, prepare, and communicate appropriate reports.

In addition, this position requires travel between sites and the ability to interact with others in both small and large group settings. At times, this position may include periods of extended physical activity, such as walking/standing at community events or to perform outreach to clients.
